摘　　要：目的应用实时三维超声心动图(RT-3DE)和斑点追踪技术(STI)评价Ⅱ型糖尿病(2DM)患者无心血管疾病症状期左房(LA)结构和功能的变化。方法收集37例2DM患者和30例健康对照者,应用RT-3DE获取LA最大容积(Vmax)、最小容积(Vmin)及收缩前容积(Vpre A),并计算LA总射血分数(TEF)、膨胀指数(EI)、主动射血分数(AEF)及被动射血分数(PEF)。应用STI获取LA负向峰值应变(NS)、正向峰值应变(PS)。比较两组统计学差异。结果 2DM组LAVmax、LAVmin及Vpre A显著增加,LATEF、LAEI及LAPEF显著减低;LAAEF显著增加,LANS绝对值显著增加,LAPS值显著减低。结论 2DM患者在无心血管疾病症状期LA的结构和功能已受到影响,进而影响LA对左室舒张功能的代偿调节作用。Objective To evaluate the changes in left atrial （LA） structures and functions with type 2 diabetes mellitus（2DM） without symptomatic cardiovascular disease by real-time three-dimensional echocardiography（RT-3DE） and speckle tracking imaging（STI）. Methods 37 patients with 2DM and 30 normal control subjects were studied. RT-3DE was used to obtain LA maximum volume（LAVmax）, LA minimum volume（LAVmin） and before atrial contraction volume（Vpre A）. LA total emptying fraction（TEF）, LA expansion index（El）, LA active emptying fraction （AEF） ,LA passive emptying（PEF） were calculated. Our study use STI to obtain LA peak positive strain （PS） and peak negative strain （NS）. The differences of two groups were compared. Results LAVmax, LAVmin and Vpre A in diabetic patients were significantly higher compared with controls. LATEF, LAEI, and LAPEF were found to be significantly lower in diabetes mellitus,while LAAEF were found to be significantly higher. STI showed LANS absolute value were higher and LAPS value were lower significantly in diabetes mellitus. Conclusions LA structure and function have been effected in 2DM patients without symptomatic cardiovascular disease. LA＇ compensatory adjustment function for left ventricular diastolic dysfunction also have been impacted.
