肺结核合并糖尿病患者的临床特点及抗结核治疗的疗效观察

摘　　要：目的:探讨肺结核合并糖尿病患者的临床特点及抗结核治疗的疗效。方法:按照是否合并糖尿病将98例肺结核患者分为实验组(肺结核合并糖尿病)50例和对照组(单纯肺结核)48例,比较2组实验室、X线胸片检查结果和临床疗效。结果:实验组患者痰涂片及培养结核杆菌(TB)阳性率、结素纯蛋白衍生物(PPD)皮试阳性率、结核抗体(TB-Ab)阳性率、血脂异常率均明显高于对照组(P<0.05);2组患者干酪浸润病灶、纤维增殖病灶发生率比较,差异无统计学意义(P>0.05);实验组患者空洞形成率明显高于对照组,病灶位于上肺率明显低于对照组(均P<0.05);实验组患者治疗2个月、6个月后痰菌转阴率、浸润病灶吸收率及空洞缩小率均明显低于对照组(均P<0.05)。结论:实验组患者的痰涂片、培养TB菌阳性率、PPD皮试阳性率、TB-Ab阳性率、血脂异常率、空洞形成率均增高,病灶部位多位于上肺等特点,与对照组治疗效果比较,在治疗2个月、6个月时痰菌转阴率、浸润病灶吸收率及空洞缩小率较差。Objective： To explore the clinical features of the patients with pulmonary tuberculosis combined with diabetes and the clinical efficacy of antituberculosis. Methods： Ninety-eight cases of pulmonary tuberculosis were divided into experimental group（ 50 cases,pulmonary tuberculosis combined with diabetes） and control group（ 48 cases,simple tuberculosis）. The results of laboratory,chest X-ray plain film findings,and clinical efficacy were compared. Results： The TB bacteria positive rate,PPD positive rate of skin test,TB-Ab positive rate,and dyslipidemia rate in the experimental group were significantly higher than those in the control group（ P〈0. 05）. There was no significant difference in the cheese infiltration lesions,and the incidence of fiber hyperplasia lesions between two groups（ P〉0. 05）. The rate of cavity formation in the experimental group was significantly higher than that in the control group（ P〈0. 05）. The proportion of the lesion located on the upper lung in the experimental group was significantly lower than that in the control group（ P〈0. 05）. The sputum negative conversion rate,infiltrating lesions absorption rate,and reduced rate of lung cavity in the experimental group were significantly lower than those in the control group（ P〈0. 05）. Conclusion： The patients with pulmonary tuberculosis combined with diabetes presented the features such as higher TB bacteria positive rate,PPD positive rate of skin test,TB-Ab positive rate,dyslipidemia rate,and lung cavity formation rate,and more lesions in the upper lung. The sputum negative conversion rate,infiltrating lesions absorption rate,and reduced rate of lung cavity are low.
