不同体位下全麻对青光眼合并宫颈癌根治术患者影响的调查研究

摘　　要：目的分析达芬奇机器人(da Vinci)手术系统行宫颈癌根治术头低体位的不同角度对合并青光眼患者的影响。方法采用回顾性分析方法,调取纳入患者的资料填写调查量表,调查量表由经培训的专人填写。调查对象为40例行达芬奇机器人青光眼合并宫颈癌根治术的患者。结果统计资料显示,心率变化:两组患者的心率在P0和P_1时点无明显的变化,差异无统计学意义(P>0.05);在P_2和P_3时点心率的变化Ⅰ组明显快于Ⅱ组,差异有统计学意义(P<0.05);患者术后4小时的眼压变化:Ⅰ组患者术后的眼压变化明显高于Ⅱ组,且有两位患者出现青光眼的症状,对照组中青光眼的发生率为10%。结论在全身麻醉下,青光眼合并宫颈癌根治术头低体位的不同角度对合并青光眼患者是有影响的,相对较小的角度安全性越高。Objective To analysis the effect of the different body angles of head-down body position in the da Vinci opera-tion system on patients with glaucoma combined with radical hysterectomy. Methods The data of 40 cases of patients with da Vinci glaucoma combined with radical hysterectomy were retrospectively analyzed, and the survey scale was filled out by the trained people. Results The statistical data showed that in terms of the heart rate change, there was no obvious change of the heart rate at the P0 and P1 time points, and the difference had no statistical significance（P〉0.05）; the heart rate changes at the P2 and P3 time points in the I group were obviously faster than those in the II group, and the differences had statistical significance（P〈0.05）, in terms of the intra-ocular tension changes at 4h after operation, the intra-ocular change after operation in the I group was obviously higher than that in the II group, and the glaucoma symptoms occurred to two patients, and the incidence rate of glaucoma in the control group was 10%. Conclusion In the general anesthesia, the dif-ferent body angles of head-down body position can have an effect on patients with glaucoma combined with radical hys-terectomy, and the safety of relatively smaller angle is high.
