15例电焊工工人胸部高千伏摄片和胸部HRCT随诊两年探讨

摘　　要：目的探究分析电焊工工人胸部高千伏摄片和胸部HRCT的改变情况。方法随机选取2013年—2014年某企业15例电焊工工人为研究对象,对其高千伏摄片和胸部HRCT的改变情况进行跟踪调查,随访两年,观察工人离岗两年后的肺部影像学变化情况。结果 4例电焊工工人肺部纤维支气管镜活检及灌洗术检查显示,灌洗液细胞包括嗜酸粒细胞、淋巴细胞、组织细胞、纤毛柱状上皮细胞等,肺泡间隔及细支气管周围可见粉尘沉积以及铁染色阳性的组织细胞沉积。高千伏摄片和胸部HRCT检查结果显示,工人两肺纹理增多,部分紊乱,两肺视野内可见弥漫性磨玻璃样密度影,其间可见少许散在类圆形小阴影。随诊两年后,离岗电焊工人员肺纹理基本恢复正常,少许散在圆形小阴影、小斑点、絮状模糊影症状基本消失,可见尘肺症状显著改善。结论电焊工工人在离岗后,可以显著改善其肺纹理紊乱情况,有助于缓解尘肺症状。Objective To study and analyze the change of chest high-kV photography and chest HRCT in 15 cases of welders. Methods 15 cases of welders in some enterprise from 2013 to 2014 were randomly selected, and the changes of high-kV photography and chest HRCT were followed-up for two years, and the changes of lung imaging in 2 years after workers were demobilized were observed. Results The lung bronchofiberscope biopsy and lavage examination of 4 cases of welders showed that the lavage fluid cells included the eosinophilic granulocyte, lymphocyte, histiocyte and ciliated colum-nar epithelial cell, and the dust deposits and iron staining positive histiocyte deposits could be seen in the alveolar septum and around the bronchioles, and the high-kV photography and chest HRCT examination results showed that the lung-markings of workers increased, and some were disordered, and the diffuse ground-glass opacity could be seen in the two-lung visual field, and a few roundlike opacities could be seen, and after two-year follow-up, the lung marking of welders leaving post returned to normal basically, a few small opacities, small spots and cotton-like blurry basically disappeared, which showed that the pneumoconiosis symptoms were obviously improved. Conclusion The lung marking disorders of the welders can be obviously improved after they are demobilized, which contributes to relieving the pneumoconiosis symptoms.
