江西省全球基金项目地区定点医院模式下结核病患者满意度分析

摘　　要：目的了解江西省全球基金项目地区定点医院模式下结核病患者的满意度,为定点医院模式下结核病控制体系的改进提供依据。方法选取江西省全球基金结核病项目2009年10月启动定点医院模式的4个县区定点医院作为研究现场,通过病案查询了解结核病患者诊疗信息,通过问卷调查了解患者满意度,采用构成比对患者的满意度进行统计描述。结果共发放问卷80份,收回有效问卷80份,问卷有效率为100%。患者对定点医院模式的8个方面(对结核病门诊环境满意度、对医护人员接诊服务态度满意度、对就诊流程方便性满意度、对医生技术水平信任度、对医生介绍肺结核知识满意度、对医生介绍知识与自身坚持服药影响度、结核病治疗费用承受度、愿意推荐亲友到定点医院就诊比例)满意度(或持正向观点比例)均在75%以上,其中愿意推荐亲友到定点医院就诊比例最高(91.25%),对定点医院结核病门诊环境满意度最低(75.00%),其余6个方面介于二者之间。结论江西省全球基金项目地区定点医院模式下的结核病控制体系是可行的,但要在全省顺利实行定点医院模式下的结核病控制体系,需注意不能加重患者经济负担,要提高患者治疗依从性,加强结防机构与定点医院之间的沟通协作等。Objective To understand the satisfaction degree of TB patients on designated hospitals model in Global Fund TB Project Areas in Jiangxi province, and to provide references for improvement of tuberculosis control system on designated hospitals model. Methods Four designated hospitals in which designated hospitals model launched in October 2009 in Global Fund TB program area were enrolled in this study. TB patients＇ information of diagnosis and treatment were attained through medical record and satisfaction degree through questionnaires. The satisfaction degree of patients was statistically described by the composition ratio. Results 80 questionnaires were sent to TB patients and 80 effective questionnaires were retrieved, and the effectvie rate of questionnaires was 100%. Satisfaction degrees in 8 as- pects of TB patients in the model of hospitals were over 75%, including TB clinic environmental satisfaction degree, medical admissions service satisfaction degree, satisfaction degree with the convenience of treatment process, level of trust in medical technology, satisfaction degree on knowledge of pulmonary tuberculosis which doctors introduced, the knowledge that doctors introduced which can influence their adhere to medication, TB treatment cost tolerance and will- ingness to recommend relatives and friends to the hospital for treatment. Among those 8 aspects, willingness to recom- mend relatives and friends to the hospital for treatment was highest （91.25%）, TB clinic environmental satisfaction was lowest （75.00%）, and satisfaction degrees in other 6 aspects were between the two. Conclusion TB control system on designated hospitals model in the Global Fund Project areas of Jiangxi province is feasible, and to successfully implement TB control system on designated hospitals model, we can not aggravate the economic burden of patients, and should improve the treatment compliance of TB patients, and strengthen the communication and cooperation between the TB control institutions and hospitals, and etc.
