摘 要:目的比较分析酮咯酸氨丁三醇与地佐辛在甲状腺手术中的镇痛效果。方法选取该院2014年8月至2015年8月收治的甲状腺手术患者120例作为研究对象,按照随机数字表法分为A、B两组,每组60例。A组患者术前给予酮咯酸氨丁三醇,B组患者术前给予地佐辛,比较两组患者拔管时间,苏醒时间,拔管后1、2、4、8 h吞咽时疼痛视觉模拟评分(VAS评分)、咳嗽和深呼吸时的布氏舒适度评分(BCS评分),以及恶心、呕吐、嗜睡、头痛、头晕等不良反应发生情况。结果 A组拔管时间和苏醒时间均明显短于B组,差异有统计学意义(P<0.05);两组患者拔管后1、2、4、8 h吞咽时VAS评分及咳嗽和深呼吸时的BCS评分比较,差异均无统计学意义(P>0.05);A组不良反应发生率[8.3%(5/60)]明显低于B组[21.7%(13/60)],差异有统计学意义(P<0.05)。结论甲状腺手术患者预先给予酮咯酸氨丁三醇与地佐辛具有较好的镇痛效果,但地佐辛较酮咯酸氨丁三醇不良反应多,拔管时间和术后苏醒时间延长。Objective To compare the analgesic effects of ketorolac tromethamine and dezocine in thyroid surgery. Methods One hundred and twenty patients with thyroid surgery in our hospital from August 2014 to August 2015 were selected as the study subjects and divided into the group A and group B according to the random number table method, 60 cases in each group. The group A was given ketorolac tromethamine before operation, while the group B received dezocine before operation. The extubation time,recovery time, VAS and BCS scores at 1,2,4,8 h after extubation and the occurrence situation of adverse reac- tions such as nausea, vomiting, drowsiness, head ache and dizziness were compared between the two groups. Results The extuba- tion time and recovery time in the group A were significantly shorter than those in the group B ,the difference was statistically sig- nificant(P〈0.05) ;the VAS and BCS scores at ! ,2,4,8 h after extubation had no statistical difference between the two groups(P〉 0.05) ;the incidence rate of adverse reactions in the group A was 8.3% (5/60), which was significantly lower than 21,7% (13/60) in the group B, the difference was statistically significant (P〈0.05). Conclusion Giving ketorolac tromethamine and dezocine in advance in the patients undergoing thyroid surgery has better analgesic effect,but dezocine has more adverse reactions than ketoro- lac tromethamine, prolongs the extubation time and postoperative recovery time.
