穴位与心理疏导用于分娩镇痛的疗效

摘　　要：目的探讨穴位与心理疏导法联合用于分娩镇痛的效果。方法正常单胎分娩产妇320例随机分成两组,观察组与对照组,观察组用心理疏导和穴位治疗法,即大肠俞穴注射无菌用水、合谷穴按摩。对照组只用心理疏导法,观察镇痛起效时间、镇痛效果、持续时间、产程时间、分娩方式、新生儿Apgar评分、产后出血量。产程进程中不同阶段的5-羟色胺、β-内啡肽、肾上腺素、去甲肾上腺素测定。应用SPSS19.0软件,所获数据采用方差分析,t检验和X^2检验。结果观察组起效时间最短2.5 min,最长5 min,止痛持续时间平均4.15 h。两组镇痛效果比较,X^2=166.000,P<0.005。两组活跃期时间和总产程时间比较,t=8.2783,5.9257;P均<0.001。观察组宫口开2 cm与开全时比较t=5.9184~17.060,P均<0.005。对照组宫口开2 cm与开全时β-EP、N比较,P<0.005。两组宫口开全时,t=4.0557~17.674,P均<0.005。其他均无显著性差异。结论穴位刺激与心理疏导在分娩镇痛机制中启动了内源性镇痛系统;缩短了产程、减轻了痛苦,增加了母婴的安全性是一种理想的分娩镇痛辅助措施。Objecive To study the acupuncture points with psychological counseling method used for labor analgesia effect. Methods 320 cases with normal single childbirth puerpera randomly divided into two groups, the observation group and control group and observation group with psychological counseling and acupuncture therapy, e. shenshu point injection of sterile water, valley, acupuncture point massage. The control group only psychological counseling method, to observe the analgesic effect and analgesic effect, duration time, lasting time, delivery mode, neonatal Apgar score, postpartum blood loss. Labor at different stages in the process of serotonin, decide - endorphins, determination of epinephrine and norepinephrine. SPSS19.0 application software, the data obtained using analysis of variance, t test and %2 test. Results observation group working time shortest 2.5 minutes, the longest five minutes, the pain lasts an average of 4.15 hours. Two groups of analgesic effect comparison, χ2 = 166.000, P〈0.005）. Two groups of active time and the total labor time, t = 8.2783, 5.9257; P〈0.001. Observation group palace mouth open 2 cm compared with open full-time t = 5.9184 - 17.060. P〈0.005. Control group palace mouth open 2 cm compared with open full-time to decide - EP, NP 〈 0.005）. Two groups of palace mouth open full-time t = 4.0557 - 17.674, P〈 0.005. There was no significant difference. Conclusion The acupuncture point stimulation and psychological counseling launched the endogenous analgesia in labor analgesia mechanism system. Shorten the labor, reduce the pain and increased the safety of maternal and infant is a kind of ideal labor analgesia auxiliary measures.
