骨科手法复位应用丙泊酚复合小剂量芬太尼的麻醉效果分析

摘　　要：目的探讨骨科手法复位应用丙泊酚复合小剂量芬太尼的麻醉效果。方法选取2014年4月至2016年5月在我院行手法复位治疗的骨折患者72例,根据麻醉方式分为观察组(采用丙泊酚复合小剂量芬太尼)和对照组(采用丙泊酚),各36例,观察并比较两组的麻醉效果。结果与麻醉诱导前相比,观察组患者麻醉诱导后1 min、手术结束时的SBP、DBP、HR、SPO2变化明显(P<0.05),对照组无明显变化(P>0.05),手术结束时两组患者的SBP、DBP、HR、SPO2比较,差异明显(P<0.05);观察组患者用药量、呼吸恢复时间、苏醒时间、手术时间均优于对照组(P<0.05);观察组患者麻醉总有效率明显高于对照组(P<0.05)。结论骨科手法复位中应用丙泊酚复合小剂量芬太尼的麻醉效果显著,是一种安全、理想的麻醉方法。Objective To investigate the anesthetic effect of propofol combined with small dose of fentanyl in manual reduction of bone fracture. Methods Seventy-two cases of bone fracture patients treated with manual reduction in our hospital from April 2014 to May 2016 was selected and divided into observation group and control group according to the anesthesia methods, with 36 cases in each group. The control group accepted propofol for anesthesia, while the observation group received propofol combined with small dose of fentanyl for anesthesia. The anesthesia effect was compared between the two groups. Results Compared with that before the induction of anesthesia, the DBP, HR, SBP and SPO2 changed significantly at 1 minutes after the induction of anesthesia and at the end of the operation in the observation group （P〈 0.05）, while the control group bad no significant changes （P〉0.05）. The differences of DBP, HR, SBP and SPO2 were significant at the end of the operation between the two groups （P〈0.05）; the dosage of anesthesia medicine, respiratory recovery time, recovery time, operation time in the observation group were significantly lower than those of the control group （P〈0.05）; the total anesthesia efficiency of the observation group was significantly higher than that of the control group （P〈0.05）. Conclusion Propofol combined with small dose of fcntanyl has a significant anesthesia effect in manual reduction of bone fracture. It is a safe and ideal anesthetic method.
