腕月骨无菌性坏死的临床进展

摘　　要：月骨无菌性坏死是由于缺血导致的月骨坏死,根据疾病的分期所采用的治疗方法虽多达20余种,但尚无任何方法能用于所有的病例,也无任何方法能使已发生碎裂的月骨恢复原有的形状和功能,传统的治疗方法主要包括保守治疗、生物学方法、生物力学的方法、月骨的切除与替代术、近排腕骨切除术、全腕关节融合术。月骨无菌性坏死病因仍然不清楚,随着对病因的研究,以期完善基因、病毒及代谢理论的发展,尽早为Kienbck’s病提供基因及分子水平的治疗方法。Kienbck＇s disease is carpal lunate bone necrosis due to ischemia. Litchman classified Kienbck＇s disease into four stages. Although the current treatments are up to 20 species,there is no any method that can apply for all cases and make the fragmentation of the lunate to restore the original shape and function. The operation approaches contain conventional treatment,biological methods,biomechanical methods,lunate bone removal and replacement,arthrodesis and proximal row carpectomy. The aetiology of Kienbck＇s disease remains unknown.Genetic,viral and metabolic theories might be developed,as they could provide avenues for genetic or molecular treatment in the initial stages of this disease.
