摘 要:目的了解肝癌患者接受I^(125)粒子植入后,不同种植粒子数的体内放射源在不同时间、不同距离对周围环境的辐射剂量率变化规律及有效防护措施。方法来自广东省人民医院接受I^(125)粒子植入术的肝癌患者共97例,依据所种植粒子数分为5组,组间人口学资料差异无统计学意义(P>0.05)。随后分别于患者术后当天、术后2个月、术后4个月,使用伽马射线检测仪检测患者肝区体表、距体表0.5 m、1 m、2 m处以及穿铅防护服时于肝区体表的辐射剂量率。结果不同种植粒子辐射剂量率组间差异有统计学意义(P<0.05);当植入粒子数<30粒时,术后当天患者体表1 m以外可视为安全距离;当植入粒子数为30~120粒时,术后当天患者体表2 m以外可视为安全距离。当植入粒子数≥120粒时,术后2个月之后2 m可视为安全距离。所有病例显示:术后患者穿着铅防护服可基本隔绝对周围环境的辐射危害;术后4个月对周围环境均无辐射危害。结论根据患者植入粒子数采取合适的隔离防护、距离防护、时间防护等措施,患者对其周围人群的辐射影响是安全的。Objective To study the change rule of radiation dose rate and effective protection measures for I 125 seeds in hepatocellular carcinoma patients with different seed numbers ,different time and different distances. Methods Ninety-seven patients from Guangdong General Hospital were divided into 5 groups according to the number of I 125 seeds, with no significant demographic differences among groups(P〉0.05). Then we tested the radiation dose rate respectively in different time and distances by using a gamma ray detector. The testing was conducted on the day of surgery, 2 months and 4 months later. The measuring position was 0 m, 0.5 m, 1 m and 2 m away respectively from the liver surface area. Results There was statistical difference of inter-group comparison of radiation dose rate (P〈0.05). The radiation safety distance was at least 1m for patients with less than 30 seeds and at least 2 m for patients with seeds ranging from 30 to 120 at the day of surgery. For patients with more than 120 seeds, the radiation safety distance was at least 2 m two months after the operation day. All cases indicated that protective clothes for postoperative patients could almost avoid the hazard radiation to the surrounding. Four months after the operation, all cases had no radiation hazard to the surrounding. Conclusion It is safe for the surrounding that the patients receive I 125 radioactive seeds implantation, provided that necessary measures have been taken, such as protective isolation, protective distance, protective time etc.
