摘 要:目的探讨人参皂苷Rh3对心肌缺血再灌注损伤(MIRI)大鼠心肌肌浆网Ca2+-ATP酶(SERCA)表达的影响。方法雄性SD大鼠96只随机分为假手术组、模型组、低[人参皂苷Rh3 4mg/(kg·d)]、中[人参皂苷Rh3 8mg/(kg·d)]和高[人参皂苷Rh3 16mg/(kg·d)]剂量组以及利多卡因[8mg/(kg·d)]组。后4组造模前连续7d给予相应药物后,建立MIRI模型,计算心肌梗死面积,观察病理学改变、4′,6-二脒基-2-苯基吲哚和TUNEL荧光染色凋亡蛋白表达,ELISA法检测TNF-α和白细胞介素6(IL-6)水平,RT-PCR和Western blot检测心肌组织SERCA mRNA和蛋白表达。结果与假手术组比较,模型组心肌细胞损伤明显,细胞核有大量凋亡蛋白分泌,低、中、高剂量组及利多卡因组凋亡蛋白分泌较少;心肌组织和血清炎性因子TNF-α和IL-6明显升高(P<0.05);低、中、高剂量组及利多卡因组上述指标则明显低于模型组(P<0.05);与假手术组比较,模型组心肌组织SERCA含量明显减少(P<0.05),与模型组比较,高剂量组的含量明显增高(P<0.05)。结论人参皂苷Rh3预处理后,可明显减轻MIRI,改善心功能、降低炎性反应,与其可以降低凋亡蛋白,增加SERCA表达有关。Objective To study the effect of ginsoside Rh3 on expression of SERCA Ca^2+-ATP in rats following myocardial I/R injury. Methods Ninety-six male SD rats were randomly divided into control group,model group,4 mg/(kg · d) ginsoside Rh3 group (group A),8 mg/(kg · d) ginsoside Rh3 group (group B),16 mg/(kg · d) ginsoside Rh3 group (group C),and 8 mg/(kg · d) lidocaine intervention group (group D). A myocardial I/R injury model of rats was established after the animals were given drugs for 7 days. Myocardial infarction size was measured. Expression of apoptotic protein was detected with HE, DAPI and TUNEL staining. Expressions of SERCA mRNA and protein in myocardial tissue were detected by RT-PCR and Western blot respectively. Results The injury of cardiac muscle cells was severer,the apoptotic protein secretion volume was larger in model group than in control group, the apoptotic protein secretion volume was smaller and the serum TNF-α and IL-6 levels were significantly higher in groups A, B, C, D than in control group and model group (P〈0.05). The expression level of SERCA was significantly lower in model group than in control group (P〈0.05) and was significantly higher in 16 mg/(kg · d) ginsoside Rh3 group than in model group (P〈0.05). Conclusion Ginsenoside Rh3 pretreatment can effectively alleviate myocardial I/R injury,improve heart function and reduce inflammatory reactions by reducing the opoptotic protein and increasing the expression of SERCA.
