摘　　要：目的通过研究前列腺增生症合并慢性前列腺炎患者与单纯前列腺增生症患者组织病理表现,探讨前列腺增生症伴发慢性前列腺炎的病理特点及治疗方案的选择。方法选取283例本院就诊并接受经尿道等离子前列腺电切术的患者,术前按泌尿外科诊治指南标准进行实验室与临床检查,筛选出合并ⅢB型前列腺炎患者66例,随机选择30例单纯前列腺增生症患者为对照组。按NICKEL的前列腺炎症程度和主要炎症类型的诊断标准对前列腺组织的病理分型与分级进行对照研究。结果前列腺增生症合并ⅢB型前列腺炎患者组织炎症表现为:前列腺腺体以轻度炎症为主,腺周与基质以中、重度炎症为主;单纯前列腺增生症患者的腺体、腺周和基质中均为轻度炎症。两组患者腺体炎症程度比较差异无统计学意义(P>0.05),两组腺周、基质炎症程度比较差异有统计学意义(P<0.05)。结论前列腺增生伴发ⅢB型前列腺炎患者病理炎症较重,炎细胞主要分布在前列腺腺周与基质,明确诊断较困难,手术治疗需谨慎。Objective To study the pathological features and treatment programs of benign prostatic hyperplasia （BPH） complicated with chronic prostatitis. Methods A total of 283 cases of BPH to undergo transurethral bipolar plas-makinetic prostatectomy （TUPKP） were involved. All patients received laboratory and clinical examinations according to the urological treatment guidelines before operation. Among them 66 were BPH with Ⅲ B prostatitis. Another 30 cases of simple BPH were enrolled as the control group. The degrees of inflammation and pathological types of the prostate tissue were analyzed according to the prostatitis diagnostic criteria and NICKEL prostatitis degree. Results BPH patients with Ⅲ B prostatitis showed： mild inflammation in prostate gland, moderate to severe inflammation in peri-gland and matrix. Simple BPH showed mild inflammation in the prostate gland, peri-gland and matrix. There was no significant difference in the degree of inflammation in the gland between the two groups （P〉0.05）, but there was difference in peri-gland and matrix （P〈0.05）. Conclusion Patients with BPH complicated with prostatitis showed severe inflammation. The inflammatory cells were mainly distributed in peri-gland and matrix. Much attention should be paid to the diagnosis and surgical treatment.
