高效抗逆转录病毒联合免疫调节对艾滋病患者炎性因子及免疫功能的影响

摘　　要：目的探讨高效抗逆转录病毒联合免疫调节对艾滋病患者炎性因子及免疫功能的影响。方法选择2011年8月至2014年12月湖北省黄冈市疾病预防控制中心收治的艾滋病患者38例为研究对象,根据入院时间先后顺序随机分为观察组（n=19）和对照组（n=19）。对照组患者接受HAART治疗（齐多夫定300 mg、拉米夫定150 mg、奈韦拉平200 mg口服,2次/d）,观察组患者在接受HAART治疗的基础上联合胸腺五肽治疗,两组均连续治疗24周。比较两组患者炎性因子、免疫功能、生活质量。结果治疗前两组患者炎性因子、免疫功能及生活质量等方面比较差异无统计学意义;治疗后均有不同程度的改善,与治疗前比较差异均有统计学意义,且观察组患者血清白细胞介素-17（IL-17）、白细胞介素-23（IL-23）含量明显高于对照组,C反应蛋白（CRP）含量明显低于对照组（P〈0.05）;CD4^＋T细胞、Th17细胞含量明显高于对照组,CD8^＋T细胞含量、Treg细胞明显低于对照组（P〈0.05）;生活质量：观察组心理、独立等评分明显高于对照组（t=2.078-3.359,P〈0.05）。结论高效抗逆转录病毒联合免疫调节能够增强免疫功能,减轻炎症反应,改善生活质量。Objective To study the effect of highly active antiretroviral therapy combined with immunomodulation on inflammatory cytokines and immune function of AIDS patients. Methods 38 cases of AIDS patients from August 2011-December 2014 were randomly divided into observation group（n =19） and control group（n =19）. The observation group patients received HAART combined with thymopentin therapy, and the control group patients received HAART therapy.The immune function, inflammatory cytokines and life quality were compared between two groups. Results In the observation group, IL-17 and IL-23 levels were significantly higher than those of the control group; CRP level was significantly lower than that of the control group（P〈0.05）. In the observation group, CD4^＋T cells and Th17 cells levels were significantly higher than those of the control group; CD8^＋T cell and Treg cells levels were significantly lower than those of the control group（P〈0.05）. Psychology and independent score of the observation group were significantly higher than those of the control group（P〈0.05）. Conclusion Highly active antiretroviral therapy combined with immunomodulation could enhance immune function, and reduce inflammation,and improve life quality of AIDS patients.
