免疫组化抗体的选择与优化对报告结果的影响

摘　　要：目的在临床病理结果报告中,往往出现免疫组化染色结果与会诊单位的染色结果不一致,导致诊断上产生分歧。本研究对免疫组化抗体以及染色操作等注意事项进行探讨,分析存在的问题。方法对科室20种常用、两个不同克隆号的抗体分为a、b两组。实验分4组进行对比实验,A组(a组抗体在link48);B组(a组抗体在Bench Mark XT);C组(b组抗体在link48);D组(b组抗体在Bench Mark XT)。结果A、B两组与标准片结果相符率为40%,C组染色结果相符率为75%,D组染色结果相符率为65%。a组抗体在两种机型的染色结果相符率为25%,b组抗体的相符率为60%。a、b组抗体分别在两种染色仪的染色结果对比有明显差异(30例,占37.5%)。显示b组抗体比a组的抗体敏感性及特异性更高,结果更可靠稳定。link48染色仪染色结果符合率更高。结论抗体及机型是否相配都会对染色结果产生影响,因此应该要重视彼此间的关系,提高染色的质量。Objective In clinical pathological report often,a inconsistent result of IHC and clinical staining result,often lead to different diagnosis. This study is to investigate the procedure of IHC staining,analyze the existing problems and provide solutions. Methods 20 kinds of commonly used, and with two different monoclones antibodies were chosen and divided into group a and b.Experiments were divided into four groups,group A（group a antibody in link48）,group B（group a antibody in BenchMark XT）, group C（group b antibody in link48）, group D（group b antibodies in BenchMark XT）. Results The consistency of the results of group A and B two groups with the standard was 40%,group C was 75%,and group D was 65%.For group a antibody,the consistency between the two machines was 25%; for group b antibody,the consistency was 60%.The a and b group of antibodies used in two different staining machines showed obvious different results（30 cases,37.5%）. Indicating that group b antibodies had a higher sensitivity and specificity,and the result was more reliable and stable. Link48 staining apparatus had a higher coincidence rate than Bench Mark XT.Conclusion To improve the quality of IHC staining results, we should pay attention to the relevance between antibodies and IHC autostainer.
