关节镜下肩峰成形术和肩峰下封闭治疗肩峰下撞击综合征效果观察

摘　　要：目的探讨关节镜下肩峰成形术和肩峰下封闭治疗肩峰下撞击综合征的临床效果。方法将68例肩峰下撞击综合征患者随机分为关节镜下肩峰成形术组和肩峰下封闭治疗组,各34例。两组治疗1个月、3个月时随访,采用视觉模拟评分(VAS)及美国加州大学肩袖评分(UCLA)进行疗效评定。结果治疗前,两组患者VAS及UCLA评分均差异无统计学意义(P>0.05)。治疗后1个月,两组VAS评分均低于治疗前(P<0.01),治疗后3个月VAS评分均低于治疗后1个月(P<0.01);两组治疗后1个月及3个月同期比较,肩峰成形术组VAS评分均低于封闭治疗组(P<0.01)。两组治疗后1个月UCLA评分均高于治疗前(P<0.01),治疗后3个月UCLA评分均高于治疗后1个月(P<0.01);两组治疗后1个月及3个月同期比较,肩峰成形术组UCLA评分均高于封闭治疗组(P<0.05或P<0.01)。结论采用关节镜下肩峰成形术治疗肩峰下撞击综合征效果更为明显。Objective Comparative analysis of arthroscopic acromioplasty and subacromial closed treatment on patients with subacromial impingement syndrome（SIS）. Methods Sixth-eight cases of SIS were divided into arthroscopic acromioplasty group and subacromial closed treatment group, they were evaluated the effect of treatment 1 months ,3 months after treatment, using visual analogue scale （VAS） and the United States University of California shoulder sleeve score （UCLA）. Results The difference of both groups was not statistically significant（ P 〉 0.05 ） before treatment, and the VAS score decreased significantly more 3 month than l months after treatment, the difference had statistical significance （P 〈 0. 01 ） ; for 1 month and 3 months, VAS score of arthroscopic acromioplasty group was lower than that of the subacromial closed treatment group, the difference had statistical significance （ P 〈 0.01 ） ; UCLA score of arthroscopic acromioplasty group increased more than that of subacromial closed treatment group as time went, and the difference was statistically significant （ P 〈 0. 01 ） ; for 1 month and 3 months after treatment, UCLA score of arthroscopic acromioplasty group was sig- nificantly higher than that of closed treatment group, the difference had statistical significance （P 〈 0. 05 or P 〈 0. 01 ）. Conclusion Arthroscopic aeromioplasty for subacromial impingement syndrome, its effect is more obvious.
