摘 要:目的:对糖尿病患者尿糖水平对尿液白细胞干化学法测定的影响及尿液中炎性细胞因子的变化进行分析研究,确定尿糖对尿白细胞干化学的影响与程度及其与尿液炎性细胞因子变化的相关性。方法:收集2014-12-2015-04尿液常规标本,用显微镜镜检筛选出尿液白细胞阳性的病例,根据尿糖结果分为尿糖阴性组(-)、阳性组(微量组±、1+组、2+组、3+组和4+组),分析这6组标本尿液干化学白细胞检测结果的差异,设计实验进行验证,同时检测各组的炎性细胞因子的变化,作相关性分析。结果:尿糖阴性组与阳性组的白细胞干化学检查结果存在显著差异(P<0.05),随着尿糖浓度升高到2+(28 mmol/L)及以上时,差异越显著(P<0.05);且随着尿糖浓度升高,干化学法白细胞检测的假阴性率越高(P<0.05),此时炎性细胞因子亦增高(P<0.05)。结论:尿糖浓度升高(28mmol/L及以上时)会对尿液白细胞干化学法检查结果产生明显影响,在临床检验工作中,对尿糖阳性(特别是尿糖28mmol/L及以上)的样本应及时显微镜复检,以确保尿液白细胞检测结果的准确性。Objective:To explore the influence of urine glucose level on the urine leukoeytes count detected by urine dry chemistry analyzer and the relevance between urine glucose level and concentrations of inflammatory factors in the patients with diabetes mellitus. Method: Taking sediment microscopy as the standard, the leukocytes count were analyzed in the urine specimens collected in Zhongnan Hospital of Wuhan University from December 2014 to April 2015. Those which contained leukocytes and the result of microscopy was positive were also detected the glucose level. According the urine glucose level, the specimens were divided into six groups, as negative, trace, 1+, 2+, 3+ and 4+. The leukocytes count was detected by urine dry chemistry analyzer, and the concentrations of inflammatory factors were also analyzed. Result:The false negative rate of detecting leukocytes count by urine dry chemistry analyzer in the group whose urine glucose was negative was lower than that of other five groups (P〈0.05). With increase of the amount of urine glucose, the false negative rate was higher and higher (P〈0.05). Especially when the urine glucose level was more than 28 mmol/L (or 2+), the difference between the negative and positive groups of urine glucose was significant (P〈0.05). Meanwhile the concentrations of inflammatory factors also increased (P〈0.05), Conclusion: Elevated urine glucose level (28 mmol/L or above) will obviously impact on the leucocytes count detected by dry chemistry method. So for the samples which urine glucose was positive (especially more than 28 mmol/L), the timely microscopy is necessary to ensure the accuracy of urine leucocytes testing.
