血浆Lp-PLA2与尿AD7c-NTP联合诊断AD的初步研究

摘　　要：目的探析血浆脂蛋白相关磷脂酶A2(Lp-PLA2)与AD相关神经丝蛋白(AD7c-NTP)联合检测在阿尔茨海默病(AD)诊断中的应用价值。方法选择西安医学院第二附属医院检验科与西安市北方医院神经内科收治53例AD患者作为观察组,另从同期来进行健康体检者中随机抽取53例作为对照组,比较两组血浆Lp-PLA2与尿AD7c-NTP水平,并计算单纯LpPLA2与尿AD7c-NTP诊断AD的诊断率及二者联合诊断率。结果观察组Lp-PLA2与AD7c-NTP水平均显著高于对照组(P﹤0.01);Lp-PLA2诊断AD的诊断率为71.7%(38/53),AD7c-NT为75.5%(40/53),均低于联合诊断的92.5%(49/53)(P﹤0.05)。结论 L血浆Lp-PLA2与尿AD7c-NTP联合诊断AD可有效避免漏诊,实现早期确诊,为患者预后改善奠定基础。Objeotive To explore the application value of plasma lipoprotein associated phospholipase A2 （Lp-PLA2） combined with AD associated neurofilament protein （AD7c-NTP） in the diagnosis of Alzheimer＇s disease （AD）. Methods A total of 53 AD patients who were admitted to Department of Laboratory of the Second Affiliated Hospital of Xi＇an Medical College and Department of Neurology in Xi＇an North Hospital were selected to constitute an observation group; another 53 persons who underwent physical examination during the same period were randomly selected to constitute a control group. Plasma Lp-PLA2 and urinary ADTc-NTP levels in the two groups were compared. The diagnostic rates of simple Lp-PLA2 and urinary AD7c-NTP and the combination in AD were calculated. Results The levels of Lp-PLA2 and AD7c-NTP in the observation group were significantly higher than those in the control group （P 〈 0.01）. The accuracy rate of Lp-PLA2 in diagnosis of AD was 71.7% （38/53） while that of urinary AD7c-NT was 75.5% （40/53） which were lower than that of the combined diagnostic rate [92.5% （49/53）] （P 〈 0.05）. （？,aaeluaion The diagnosis of AD by Lp-PLA2 combined with AD7c-NTP can effectively avoid missed diagnosis, realize early diagnosis and lay foundation for the improvement of the prognosis of patients.
