摘　　要：目的探析联合治疗对不同烟龄哮喘患者炎症因子及肺功能的作用。方法以我院收治的78例支气管哮喘患者为研究对象,根据患者烟龄不同分为A组(烟龄﹤3年,46例)及B组(烟龄≥3年,32例)。两组均应用沙丁胺醇气雾剂、布地奈德福莫特罗联合阿奇霉素片进行治疗,比较两组治疗前后诱导痰炎症因子(ICAM-1、VEGF、IL-13、IL-17)水平和肺功能(FEV1、PEF),记录两组不良反应。结果治疗前两组炎症因子和肺功能均无统计学差异(P﹥0.05);治疗后,A组炎症因子水平低于治疗前及B组治疗后,而肺功能高于治疗前及B组治疗后(均P﹤0.05)。两组治疗期间均未出现严重不良反应,治疗未中断。结论沙丁胺醇气雾剂、布地奈德福莫特罗联合阿奇霉素治疗哮喘,可降低患者气道炎症因子水平,抑制气道炎症反应,促进肺功能恢复,但长期吸烟会导致支气管哮喘患者对治疗的反应性降低。Objective To explore the effects of combination therapy on inflammatory factors and lung function in patients with asthma and different smoking histories. Methods A total of 78 patients with bronchial asthma admitted to our hospital were selected and divided into group A （smoking history 〈 three years, n = 46） and group B （smoking history 1〉 three years, n = 32） according to their smoking histories. The two groups were treated with salbutamol aerosol, budesonide/formoterol combined with azithromycin tablets to compare the levels of inflammatory factors （ICAM-1, VEGF, IL-13 and IL-17） in induced sputum and lung function （FEV1 and PEF） before and after the treatment. The adverse reactions were recorded. Results There was no significant difference in the inflammatory factors and lung function between the two groups before the treatment （P 〉 0.05）; after the treatment, the levels of the inflammatory factors in group A were lower than those in group B, but the lung function was better than that before the treatment and that in group B （P 〈 0.05）. No severe adverse reaction occurred during the treatment in the two groups and the treatment was not interrupted. Conclusion The treatment of asthma with salbutamol aerosol, budesonide/formoterol in combination with azithromycin tablets can lower the levels of airway inflammatory factors, inhibit airway inflammatory response and promote the recovery of lung function.
