心莲清肺汤联合喜炎平注射液对小儿细菌性肺炎患者PCT、CRP的影响

摘　　要：目的探究心莲清肺汤联合喜炎平注射液对小儿细菌性肺炎患者血清C反应蛋白(C reactive protein,CRP)、降钙素原(procalcitonin,PCT)水平的影响。方法研究对象为经确诊的78例小儿细菌性肺炎患儿,随机分成2组,每组39例。常规给予对症处置后,对照组患儿予心莲清肺汤治疗,观察组患儿在上述治疗基础上再予喜炎平注射液静点,治疗前后血清感染性炎症指标,记录症状及体征消失时间和住院时间以及肺炎症状评分,观察临床疗效及安全性。结果与治疗前比较,2组血清CRP、PCT、白细胞介素(IL-6)水平以及白细胞计数(WBC)计数均降低(P<0.01);与对照组比较,观察组患儿血清CRP、PCT、IL-6水平以及WBC计数均较低(P<0.01),治疗后1、3、5、10d肺炎症状评分均显著性较低(P<0.01),退热时间,肺部罗音、咳嗽消失时间以及住院时间均显著较短(P<0.01),治疗有效率较高(P<0.01),不良反应率差异无统计学意义。结论心莲清肺汤联合喜炎平注射液对小儿细菌性肺炎有确切疗效,显著缓解肺炎临床症状及体征,改善机体感染情况,缩短患儿住院时间,并有较好的安全性。Objective To explore the clinical effect of Xinlian Qingfei decoction combined with Xiyanping injection in childern with bacterial pneumonia in serum C reactive protein（CRP） and procalcitonin（PCT）. Methods 78 cases of children with bacterial pneumonia were randomly divided into 2 groups,39 cases in each group. Patients received conventional symptomatic treatment,and the control group were treated with Xinlian Qingfei decoction,and the experimental group were treated on the basis of the control group with Xiyanping injection. The inflammation indexes were recorded,symptoms and signs disappeared time,length of stay and pneumonia symptom score,and clinical effect,safety were compared. Results Compared with before treatment,serum CRP,PCT,IL-6 and WBC decreased（P〈0. 01）; and compared with the control group,serum CRP,PCT,IL-6 and WBC count in the experimental group were lower（P〈0. 01）,the pneumonia symptom scores after 1,3,5,10 days＇ treatment were significantly lower（P〈0. 01）,the time of fever,pulmonary rales,cough disappearance time and hospitalization time were significantly shorter（P〈0. 01）,and the effective rate was higher（P〈0. 01）,the adverse reaction rate had no significant difference. Conclusion Xinlian Qingfei decoction combined with Xiyanping injection in the treatment of bacterial pneumonia in children has exact curative effect,significantly alleviated clinical symptoms and signs of pneumonia,improve body infection and shorten the time of hospitalization with good safety.
