钬激光碎石术在腹腔镜下胆道探查中的应用体会

摘　　要：目的探讨腹腔镜技术结合钬激光碎石治疗肝内外胆管结石的治疗效果及体会。方法回顾性分析2008年7月至2015年12月,采用纤维胆道镜下钬激光碎石术治疗肝内外胆管结石患者232例的临床资料,激光治疗功率6.4-9.6W,频率8Hz,能量800-1200mJ,观察患者的临床疗效和不良反应发生率。结果术中取石取净率72.3%（89/123）,一期胆管缝合率37.4%（46/123）,降低了胆管结石的残留率。结论腹腔镜胆道探查术中使用钬激光碎石术能有效地治疗肝内外胆管结石。Objective To analyze the application experience of the fiber optic choledochoscope-aided holmium laser lithotripsy in the treatment of intrahepatic and extrahepatic lithiasis. Methods Totally 128 patients of intrahepatic and extrahepatic lithiasis having received treatment of fiber optic choledochoscope-aided holmium laser lithotripsy from Jul. 2008 to Jul. 2012 were enrolled to this study, The therapeutic effect was analyzed. Results All of 128 patients, 121 patients had the stone removed completely. The residual cholelithes were successfully removed. The success rate of cholelithes extraction was improved. Conclusions The application of the fiber optic choledochoscope-aided holmium laser lithotripsy is effective in treating intrahepatic and extrahepatic lithiasis.
