蛋白C、N末端前体脑钠肽水平与热射病患者危重程度相关性分析

摘　　要：目的探讨蛋白C、N末端前体脑钠肽(NT-ProBNP)水平与热射病患者急性生理与慢性健康状况评分(APACHEⅡ评分)的相关性。方法回顾性分析2012年1月至2016年8月沈阳军区急诊医学部收治的热射病患者48例,根据APACHEⅡ评分将患者分为低分组、高分组。比较两组患者的蛋白C、NT-ProBNP水平及与APACHEⅡ评分的相关性。结果低分组患者的蛋白C水平明显高于高分组,NT-ProBNP水平明显低于高分组,差异均有统计学意义(P<0.01)。蛋白C与APACHEⅡ评分存在明显的负相关(P<0.01);NT-ProBNP与APACHEⅡ评分存在正相关(P<0.01);结论热射病患者蛋白C、NT-ProBNP水平与APACHEⅡ评分关系密切,可作为病情进展评价及疗效判定的指标,为临床诊疗提供新的思路。Objective To explore the correlation of protein C,NT-ProBNP and APACHE Ⅱ score in heat stroke patients.Methods A retrospective study was performed on 48 patients with heat stroke received in The General Hospital of Shenyang Military Command from January 2012 to August 2016. Patients were divided into the low score group and the high score group due to APACHE Ⅱ score. Correlation in protein C,NT-ProBNP and APACHE Ⅱ score in the two groups was compared and analyzed. Results Protein C level and NT-ProBNP in the low score group were significantly higher and lower than those in the high score group,respectively（ P〈0. 01）. There was significantly negative correlation between protein C and APACHE Ⅱ score（ P〈0. 01） and positive correlation between NT-ProBNP and APACHE Ⅱ score（ P〈0. 01）. Conclusion There is close correlation among protein C,NT-ProBNP and APACHE Ⅱ score in patients with heat stroke,which has progressive evaluation and outcome indicators,can provide new thinking for clinical diagnosis and treatment.
