228例疑似新生儿溶血病患儿相关试验结果分析

摘　　要：目的通过对本院疑似新生儿溶血病(HDN)患儿及其孕母的血清学检测,分析HDN在本院的发病率,并对相应孕母的血清Ig G抗体及效价分布进行对比分析,为临床明确诊断和治疗提供依据。方法选择2015年1月-2015年12月浙江省台州市中心医院疑似HDN患儿228例,检测患儿及其孕母血型、血清Ig G抗体与效价以及新生儿直接抗人球蛋白试验、游离试验、放散试验的溶血三项试验。结果 228例疑似HDN病例中71例确诊为HDN,占31.1%,将HDN孕母组与非HDN孕母组血清Ig G抗体效价以1∶64为界限分组,不同组别发病率差异有统计学意义(P<0.01)。结论新生儿溶血病确诊病例在本院疑似HDN病例中占比较高,要给予高度重视;对于Ig G抗体效价较高孕妇要加强动态监测,并适时进行临床干预,避免或减轻免疫性溶血病的危害,实现优生优育。Objective Through the serological detection about the suspected hemolytic diseases of the newborns（ HDN） and their mother in our hospital,to analyze the incidence rate of HDN in our hospital,and to comparatively analyze the serum Ig G antibody and titer distribution,so as to provide basis for the clinical diagnosis and treatment. Methods 228 cases of suspected HDN were selected in Taizhou Cental Hospital from January 2015 to December in 2015 for the detection of children and their mother’s blood type,the mother’s serum Ig G antibody and titer distribution,and for the neonatal directly anti globulin test,free test,and radiation test. Results In the 228 suspected HDN cases,71 cases were diagnosed as HDN,accounting for31. 1%,according to the serum Ig G antibody titer with 1 ∶64 for the grouping boundaries from the HDN or non HDN group motherhood,the difference on the incidence rates in different groups was statistically significant（ P 〈 0. 01）. Conclusion The proportion of neonatal hemolysis disease confirmed cases is higher from the suspected HDN cases in our hospital,so it has to be given high attention; the pregnant mothers with higher Ig G antibody titer must be under strong dynamic monitoring,and given timely clinical intervention,so as to avoid or mitigate the harm of autoimmune hemolytic disease,and realize the goal of eugenics.
