摘 要:目的:探讨高压氧(HBO)联合硫辛酸(ALA)治疗糖尿病周围神经病变(DPN)的疗效。方法经患者知情同意,并经医院伦理委员会批准,92例DPN患者分为HBO组和ALA组,各46例,ALA组常规降血糖治疗、硫辛酸静滴,HBO组在此基础上加用HBO治疗。肌电图检测运动神经、感觉神经传导速度、潜伏期、反应波的波幅。结果 HBO组和ALA组患者治疗后运动神经传导速度(MNCV)均加速[HBO组腓总神经治疗前:(28.74±5.45)m/s,治疗后:(36.20±1.87)m/s]、潜伏期缩短[HBO组腓总神经治疗前:(7.00±1.32)mV,治疗后:(4.87±1.18)mV]、波幅增高[HBO组腓总神经治疗前:(3.04±1.38)mV,治疗后:(4.59±0.98)mV],差异有统计学意义(P<0.05);感觉神经传导速度(SNCV)加速、波幅增高、差异有统计学意义(P<0.05)。结论 HBO联合药物治疗,在功效上起到了累加的作用,可有效减少脱髓鞘改变及轴突变性的发生,利于改善DPN患者症状、恢复周围神经的传导功能。Objective To explore the therapeutic effects of HBO coupled with thioctic acid in the treatment of diabetic peripheral neuropathy (DPN).Methods Ninety-two patients with DPN were randomly divided into the HBO group and the ALA group,each consisting of 46 patients.The patients in the ALA group received routine hypoglycemic therapy and intravenous infusion of thioctic acid,while the patients in the HBO group were supplemented with HBO in addition to the above routine treatment.EMG was used to detect motor nerve conduction velocity (MNCV)and sensory nerve conduction velocity (SNCV),latency and response wave amplitude.Results After treatment,it could be seen that MNCV[nervus peroneus communis of HBO group, before treatment:(28.74 ±5.45)m/s,after treatment:(36.20 ±1.87)m/s]was accelerated,latency was shortened and wave amplitude was elevated,with statistical significance(P〈0.05).It was quite noticeable that SNCV was accelerated,and wave amplitude was increased,also with statistical significance(P〈0.05). Regarding the treatment results,the HBO group was superior to the ALA group,and statistical significance could be noticed,when comparisons were made between them(P〈0.01 ).Conclusions HBO coupled with drug therapy could produce certain cumulative effects and effectively alleviate incidence of demyelination and axonal degeneration.So,this treatment method could improve the symptoms of patients with DPN,and facilitate quick recovery of peripheral nerve conduction.
