MRI联合TRICKS血管造影在四肢软组织病变中应用价值

摘　　要：目的探讨常规MRI联合时间分辨对比剂动态显像技术(TRICKS)血管造影术在四肢软组织病变诊断中的应用价值。方法选取赤峰市医院自2013年3月至2014年10月收治的四肢软组织病变患者134例。所有患者先行常规MRI平扫;注入对比剂后,用TRICKS技术进行动态血管造影;最后,用腹部常用的LAVA技术进行增强扫描。扫描结束后,将134例患者随机分成两组:A组行常规MRI检查(平扫+增强),B组在A组的基础上增加TRICKS血管造影,两组分别对病变进行定性诊断,并各自与手术病理进行对照。结果所有患者均顺利完成TRICKS血管造影和常规MRI检查,TRICKS血管造影能清晰显示病变区域动静脉的充盈情况和动态变化。A组诊断肿瘤与非肿瘤病变的准确性、敏感性和特异性分别为86.5%、94.3%、57.1%,B组分别为95.5%、98.1%、85.7%,两组间敏感性比较,差异无统计学意义(P>0.05),而特异性、准确性比较,差异有统计学意义(P<0.05)。A组对良恶性病变诊断的准确率、敏感性和特异性分别为83.5%、92.0%、78.5%,B组分别为94.0%、96.0%、92.9%,两组间敏感性比较,差异无统计学意义(P>0.05),而特异性、准确性比较,差异有统计学意义(P<0.05)。结论 MRI联合TRICKS血管造影用于四肢软组织病变的影像学诊断方法可行,且优于单纯MRI检查。Objective To explore the feasibility and value of magnetic resonance angiography using time resolved imaging of contrast kinetics（ TRICKS） combined with MR scanning in the diagnosis of lesions of extremity soft tissue. Methods Totally 134 cases of patients with diseases of extremity soft tissue were received in Chifeng Municipal Hospital from March 2013 to April 2014. After MRI scan,angiography was performed by using TRICKS with contrast medium,and then enhancement scan with LAVA. All cases were divided into two groups,Group A with MRI,Group B with MRI and TRICKS. The diagnosis results were compared between Groups A and B with pathological examinations. Results All cases received MRI and TRICKS angiography. From the TRICKS images,filling situation and dynamic change of the vessels in lesions of extremity could be observed clearly. Veracity,sensibility and specificity in Group A for tumor diagnose was 86. 5%,94. 3% and 57. 1%,while 95. 5%,98. 1% and 85. 7% in Group B. Veracity,sensibility and specificity in Group A for benign and malignant diagnose was 83. 5%,92. 0% and 78. 5%,while 94. 0%,96. 0% and 92. 9% in Group B. There were statistical differences between Group A and B in veracity and specificity（ P〈0. 05）,but no statistical differences in sensibility（ P〉0. 05）. Conclusion It is an effect approach for the imageological diagnosis of lesions of extremity soft tissue via magnetic resonance angiography using TRICKS combined with MR scanning,and it is better than merely MR scan.
