摘  要:目的探讨主动脉根部替换术(Bentall术)对主动脉根部病变的临床疗效。方法选择2012年3月-2015年3月新疆医科大学第一附属医院心脏外科经Bentall术治疗主动脉根部病变的160例患者,通过术前、术后的心功能评价手术疗效。结果 160例患者围手术期死亡12例,死亡率为7.5%。其中术中严重低心排综合征死亡3例(2例同期行冠脉搭桥术,1例因术前心功能差,术中停体外循环后发生低心排)。ICU重症监护期间死亡9例,其中低心排死亡5例,严重出血死亡1例,术后急性肾衰死亡1例,恶性心律失常死亡1例,纵膈感染死亡1例。148例患者术后症状较术前明显缓解或完全消失,心功能为Ⅰ~Ⅱ级。结论应用Bentall手术效果显著,患者心功能明显改善,对主动脉根部病变患者的疗效至关重要。Objective To analyze the effect of Bentall operation on 160 cases of aortic root lesions caused by various reasons. Methods 160 patients with aortic root lesions and implemented Bentall surgery in Department of Cardiac Surgery of the First Affiliated Hospital of Xinjiang Medical University from March 2012 to March 2015 were retrospective analysed. The surgical efficacy were evaluated by comparison of preoperative and postoperative cardiac function. Results 12 patients died in 160 cases during perioperative period,and the mortality rate was 7. 5%. Among them,3 patients died of severe low cardiac output syndrome( LOCS)( 2 patients had undergone coronary artery bypass grafting,1 patient had poor cardiac function before operation and low cardiac output after operation).148 patients' symptoms alleviated obviously or disappeared and their cardiac function reached Ⅰ- Ⅱ. Conclusion Bentall operation can improve the patients' cardiac function. Bentall surgery is essential for the treatment of patients with aortic root disease.
