突发性耳聋患者治疗中出现听力损失加重的病因分析

摘　　要：目的探讨突发性耳聋患者治疗中出现听力损失加重的病因。方法选择突发性耳聋治疗中出现听力损失加重的患者共42例。观察各频率听力损失加重发生率及程度,探索听力损失加重的原因,采用葛根注射液+强的松治疗后评估疗效。结果突发性耳聋患者听力损失加重在同期收治的突发性耳聋患者的比率为6.22%(42/675)。12例患者因休息欠佳或精神压力而出现治疗过程中的听力损失加重,另外30例则原因不明。12例休息欠佳或精神压力大者中低频型最为多见,30例原因不明者中较多(20例)属于平坦型。42例患者有6例痊愈,12例显效,7例有效,总有效率为59.5%。结论突发性耳聋患者在治疗过程中会出现听力损失加重的情况,其原因包括精神心理因素,部分患者可能与溶栓导致的内耳出血加重、膜迷路积水等有关。Objective To explore the etiology of exacerbation of hearing loss occurring in the treatment of patients with sudden deafness.Methods A total of 42 patients with hearing loss were collected and patients with idiopathic hearing loss occurred during treatment were selected for this study.Frequency and incidence in hearing loss and the severity of exacerbation were observed and the reasons for aggravation of hearing loss were explored.The efficacy was assessed after treatment with Gegen injection plus prednisone.Results The proportion of patients with sudden deafness and exacerbation of hearing loss was 6.22%（ 42 /675） in sudden deafness patients treated during the same period.12 patients with increased hearing loss were due to poor seating or mental stress arising during the treatment course.In addition,in comparison with 30 cases of unknown causes,12 cases of mental stress or poor rest were the most common type of low frequency.In the other 30 cases of unknown causes,the most of them（ 20 cases） belonged to the flat type.Among these 42 patients,6 patients were completely cured,12 of them were cured,and 7 patients were effective,and the total effective rate was 59.5%,and the remaining 17 patients were inactive cases,accounting for 40.5%.Conclusion Hearing loss aggravating circumstance may occur in patients with sudden deafness during the treatment course,and their reasons including psychological factors,thrombolytic inner ear bleeding and endolymphatic hydrops etc.
