大黄敷脐治疗便秘的Meta分析

摘　　要：[目的]评价大黄敷脐对便秘治疗效果的影响。[方法]计算机检索PubMed、The Cochrane Library、EMBASE、中国期刊全文数据库(CNKI)、万方数据库(WanFang Data)、维普数据库(VIP)和中国生物医学数据库(CBM),检索时限均为建库至2015年5月关于大黄敷脐治疗便秘的随机对照试验,并追溯纳入研究的参考文献。由两名研究员对文献质量进行评价。应用RevMan 5.2进行Meta分析,采用Cochrane系统评价员手册5.0.2推荐的工具评价其方法学质量。[结果]共纳入10个治疗便秘的随机对照试验,涉及850例病人,纳入的所有文献均为中文文献。Meta分析结果表明:大黄敷脐治疗便秘总有效率高于对照组[RR=1.59,95%CI(1.29,1.95),P<0.000 01];与常规护理措施相比,大黄联合薄荷脑能有效治疗便秘[RR=2.77,95%CI(1.54,4.98),P<0.000 6]。所有试验均无严重不良事件报道。[结论]大黄敷脐治疗便秘安全有效,但因缺乏高质量的研究,目前还不能得到较为可靠的结论。Objective：To evaluate the influence of umbilical area applying with rhubarb on curative effect of constipation.Methods：It searched PubMed、The Cochrane Library、EMBASE、CNKI、WanFang Data、VIP and CBM from inception to May 2015 on the rhubarb umbilical area applying for constipation in randomized controlled trials,and traced references of included studies.The literature quality was evaluated by two researchers.RevMan5.2version was used for Meta analysis.Cochrane system evaluation manual 5.0.2recommended tool was used to evaluate the quality of its methodology.Results：10randomized controlled trials for treatment of constipation with 850 patients were included.All the included literatures were Chinese literature.Meta analysis results showed that the total effective rate of rhubarb umbilical area applying was higher than that of control group[RR=1.59,95%CI（1.29,1.95）,P〈0.000 01].Compared with the conventional nursing measures,rhubarb combined with menthol umbilical area applying could treat constipation effectively[RR=2.77,CI（1.54,4.98）,P0.000 6].No serious adverse events were reported in all trials.Conclusions：Rhubarb umbilical area applying was safe and effective in treating constipation,but it could not get a reliable conclusion because of the lack of high quality research.
