血脂和CA125对肺癌和肺结核的鉴别诊断

摘　　要：目的探讨血脂和血清CA125水平对肺癌和肺结核的鉴别诊断价值。方法采用化学比色法、免疫比浊法和电化学发光免疫分析检测131例肺癌（Ⅰ-Ⅳ期）、53例肺结核病和60例健康对照者的血脂和血清CA125水平,并进行对比研究。结果131例肺癌患者血清总胆固醇（TC）和高密度脂蛋白（HDL-C）水平随疾病严重程度的增加而降低,肺癌Ⅰ-Ⅳ期TC水平与53例肺结核比较,差异均有统计学意义（P〈0.05、〈0.05、〈0.01、〈0.001）,HDL-C水平与肺结核比较,差异均有统计学意义（P〈0.05、〈0.05、〈0.01、〈0.01）;131例肺癌Ⅰ-Ⅲ期低密度脂蛋白（LDL-C）水平与53例肺结核比较,差异无统计学意义（P〉0.05）,仅肺癌Ⅳ期稍增高（P〈0.05）;131例肺癌三酰甘油（TG）和载脂蛋白B与载脂蛋白A1（ApoB/ApoA1）比值水平随严重程度的增加而增高,与53例肺结核Ⅰ-Ⅳ期TG比较,差异有统计学意义（P〈0.05、〈0.05、〈0.01、〈0.01）,ApoB/ApoA1比值Ⅰ-Ⅳ期比较,差异有统计学意义（P〈0.05、〈0.05、〈0.01、〈0.01）。131例肺癌患者血清CA125水平随肺癌的严重程度增加而增高,与53例肺结核比较,差异有统计学意义（P〈0.001）。结论血清TC、HDL-C、TG、ApoB/ApoA1、CA125可能是肺癌与肺结核病鉴别诊断的指标。Objective To explore the value of blood lipid and CA125 in identification diagnosis between lung cancer and pulmonary tuberculosis.Methods The blood lipid and serum CA125 levels in 131 patients with lung cancer（Ⅰ-Ⅳ stage）,53 patients with pulmonary tuberculosis and 60 normal controls were determined by biochemstry,immunoturbidimetry and electrochemiluminescence immunoassay and was done compared study.Results The serum TC and HDL-C level in 131 patients with lung cancer were decreased with severity and were lower than in those of in 53 patients with pulmonary tuberculosis（TC：P〈0.05,0.05,0.01 and 0.001respectively;HDL-C：P〈0.05,〈0.05,〈0.01and〈0.01respectively）.The serum LDL-C level was no differences between lung cancer（Ⅰ-Ⅲstage）and in 53 patients with pulmonary tuberculosis（P〈0.05）only the lung cancer（Ⅳstage）was a little increased（P〈0.05）.The serum TG and ApoB/ApoA1 ratio levels in 131 patients with lung cancer（Ⅰ-Ⅳstage）were increased with severity and were significantly higher than in those of 53 patients with pulmonary tuberculosis（TG：P〈0.05,〈0.05,〈0.01 and 〈0.01respectively;ApoB/ApoA1：P〈0.05,〈0.05,〈0.01and〈0.01respectively）.The level of CA125in131 patients with lung cancer was significantly higher than in those 53 patients with pulmonary tuberculosis（P〈0.001）.Conclusion The serum TC and HDL-C,TG,ApoB/ApoA1 ratio and CA125 levels may be the indexs of identification diagnosis between lung cancer and pulmonary tuberculosis.
