血清Apelin-13预测功能性心肌缺血的价值

摘　　要：目的 探讨血清Apelin-13对功能性心肌缺血患者的诊断价值.方法 共纳入77例冠状动脉造影证实为冠状动脉临界病变患者,根据心肌血流储备分数（FFR）值将患者分为FFR＜0.80组（n=36）和FFR＞0.80组（n=41）.检测所有患者入院24h内血清Apelin-13水平,比较两组患者Apelin-13值的差异性,判断其诊断功能性心肌缺血的临床价值.结果 血清Apelin-13水平值在两组患者中有明显差异[（31.822±6.703）ng/ml比（45.163±5.071）ng/ml,P＜0.01].ROC曲线分析提示,血清Apelin-13水平值在38.275 ng/ml时,预测FFR＜0.80的敏感度和特异度均为94.4％,曲线下面积0.94[95％可信区间（CI）0.870～1.007].结论 低血清Apelin-13水平与功能性心肌缺血患者相关,有一定的诊断价值.Objective To assess the value of serum Apelin-13 in predicting functional myocardial ischemia in patients with coronary borderline lesion. Methods 77 patients with coronary borderline lesion were included in this study and according to fractional flow reserve which is divided into two groups：FFR〈0.80 （n=36） and FFR〉0.80 （n=41）. Serum Apelin-13 level was measured in patients with coronary borderline lesion admitted to hospital within 24 h. Compare two groups difference to judge the clinical value of the diagnosis. Results There were significant differences of Apelin-13 level [ （ 31.822±6.703 ）ng/ml vs （45.163±5.071）ng/ml, P〈0.01 ] between the two groups. ROC curve analyses to determine the optimal cutoff value of serum Apelin-13 level for predicting functional myocardial ischemia was 38.275 ng/ml, which yielded a sensitivity of and a specificity of 94%, respectively. With the area under the curve at 0.94 （95%CI 0.870-1.007）. Conclusion Lower serum Apelin-13 level is associated with patients with functional myocardial ischemia.
