高龄CABG患者常规切开与内窥镜法采集大隐静脉下肢并发症的对比研究

摘　　要：目的 对比高龄冠状动脉旁路移植术（CABG）患者常规切开与内窥镜法采集大隐静脉下肢并发症的发生情况,评价内窥镜下采集大隐静脉用于高龄CABG患者的临床效果.方法 选取2014年6月至2015年12月于我院行冠状动脉旁路移植术高龄患者196例,分为内窥镜静脉采集组（EVH组,98例）和常规切开静脉采集组（CVH组,98例）,比较两种静脉采集方法的术后下肢并发症情况.结果 与CVH组相比,EVH组的切口长度、术后疼痛评分、术后并发症和住院时间均小于CVH组,两组比较差异具有统计学意义（P＜0.05）;两组采集的大隐静脉质量、总手术时间及住院费用等方面比较未见统计学差异（P＞0.05）.结论 应用内窥镜采集大隐静脉能够明显降低高龄患者冠状动脉旁路移植术后下肢并发症的发生率,早期效果满意.内窥镜采集大隐静作为一种微创取血管的方法,对于高龄冠状动脉旁路移植术患者是安全和值得推荐的.Objective To compare the post-operative lower extremity complications for elderly patients with coronary artery bypass grafting（CABG ） between endoscopic saphenous vein harvesting（EVH ） and conventional vein saphenous harvesting （CVH） in order to evaluate clinical application of the saphenous vein with endoscopic vascular acquisition system for elderly patients with coronary artery bypass grafting. Methods 196 cases of elderly patients who were underwent coronary artery bypass grafting from June 2014 to December 2015 in our hospital were selected and divided into endoscope vein harvesting group （EVH group ,n=98 ） and conventional vein harvesting group （CVH group, n=98 ）. Post-operative lower extremity complications of the two collecting methods were com- pared. Results Incision length, postoperative pain scores, complications and hospitalization time in EVH group were significantly lower compared to the CVH group （P〈0.05）. There were no statistical significance in the quality of GSV, total operation time and total cost of hospitalization between two groups （P〉0.05）. Conclusion EVH for elderly patients with coronary artery bypass grafting has lower extremity complications. The early efficacy of EVH undergoing CABG is satisfied. Thus it is a safe and recommendable method as a minimally invasive approach for elderly patients with coronary artery bypass grafting.
