摘  要:目的 探讨肿瘤坏死因子-alpha(TNF-α)在中华小猪急性心肌梗死(AMI)梗死心肌中的动态表达,为AMI血运重建及心肌梗死后干预提供实验依据.方法 7头实验性中华小猪全麻后Seldinger法穿刺右侧股动脉,行冠脉造影后经微导管注入明胶海绵封堵钝缘支(OM支),建立AMI模型后,分别取第1、3、5、7、10、14、17天行免疫组化检测心肌组织中TNF-α表达情况.结果 免疫组化显示,正常猪心肌未见TNF-α表达.AMI后第3天坏死心肌中开始出现TNF-α表达,第5、7天表达逐渐升高,第10天达高峰,第14天开始下降,第17天进一步下降.结论 冠脉造影显示,明胶海绵封堵OM支后其远端血流消失,AMI模型成功建立.AMI后第10天坏死心肌中TNF-α表达最为明显,与坏死心肌的炎症及修复过程密切相关.Objective To observe the dynamic expression of tumor necrosis factor alpha (TNF-α) in myocardial tissue in which Chinese small porcine survive different days and to provide experimental basis of revascularization and intervention after AMI. Methods The obtuse marginal (OM) branch of 8 Chinese small porcines were blocked by gelatin sponge via microcatheter under general anesthesia through Seldinger right femoral artery puncture after coronary angiography. After AMI model were established successfully, the animals were sacrificed after 1,3,5,7, 10, 14, 17 days respectively use immunohistochemical staining to test the expression of tumor necrosis factor alpha (TNF-el) in the cardiac muscles. Results Immunohistochemistry showed there was no TNF-α in normal myocardial tissue, TNF-α was started to express in the necrotic myocardial tissue on 3 day after AMI, increased gradually on 5-7 day post-AMI, peaked on 10 day and started to decrease on 14 day, decreased further on 17 day. Conclusion Coronary angiography showed that the remote flow disappeared after the OM branches were blocked by gelatin sponge, the model of AMI was establish successfully. The necrotic myocardial tissue was repaired preliminarily and revascularization was safe relatively. The expression of TNF-α in the necrotic areas peaked on 10 day post-AMI and were associated with the inflammation and reparative processes of the necrotic cardiac muscles.
