摘 要:目的研究CBCT三维头影测量与X线头颅侧位片二维头影测量的差异性。方法 30名正常成人同时拍摄X线头颅侧位片和CBCT。对头颅侧位片进行硫酸纸人工描图,对CBCT进行三维重建,分别进行Downs和Mc Namara分析法的18个项目的 2D头影测量和3D头影测量。由1名正畸科医生分别测量3次,间隔时间为1周。采用组内相关系数(ICC)评估2D和3D测量操作者内的复测可靠性,用配对t检验比较2D和3D测量的差异性。结果对于操作者内ICC,2D和3D的均值分别为0.962和0.959。2D和3D差异性比较中,NP/FH、NP/AB、L1/MP、L1/OP、U1-AP、Co-A、Co-Gn、ANS-Me、U1-A、Ba N/Pt Gn测量值差异有统计学意义。结论 Downs分析法和Mc Namara分析法CBCT 3D头影测量与头颅侧位片2D头影测量值存在差异。需建立一套新的三维头影测量体系以供临床医生参考。Objective To investigate the difference between conventional lateral cephalometric and cone beam computed tomography (CBCT) measurements. Methods Thirty adults with normal occlusion were selected from Harbin region. Conventional lateral cephalometric radiographs and CBCT were taken respectively. Manual tracing cephalometric measurement was performed on 2D lateral cephalometric images and the CBCT was 3D reconstructed. 2D and 3D measurements were performed by Downs and McNamara. Intraclass correlations ( ICC ) and paired t tests were used to analyze the reliability and difference. Results The mean ICC was 0. 962 for 2D and 0. 959 for 3D assessments. There was significant difference in measurements of NP/FH, NP/AB, L1/MP, L1/OP, U1-AP, Co-A, Co-Gn, ANS-Me, U1-A, BaN/ PtGn between 2D and 3D (P 〈 0. 05). Conclusion There are differences between 3D and 2D measurements. A new system for CBCT should be established.
