全髋关节置换术对成人髋关节发育不良合并股骨上端骨折的疗效及远期预后的影响

摘　　要：目的分析全髋关节置换术对成人髋关节发育不良合并股骨上端骨折的疗效及远期预后。方法选择髋关节发育不良合并股骨上端骨折的61例患者,均行全髋关节置换术。评价疗效,观察患者治疗前后的双下肢长度差、生活质量评分、疼痛感和行走情况的改善。结果治疗后,61例患者总优良率为73.77%。手术后各类型患者的Harris评分均显著提高(P<0.05),患者手术行走能力和疼痛感较手术前均有显著改善(P<0.05)。患者在手术后的双下肢长度差为(1.9±0.7)cm,显著短于手术前的(8.7±1.5)cm(P<0.05)。手术后患者的身体功能、心理功能、情感功能以及社会功能方面的生活质量评分显著高于手术前(P<0.05)。结论成人髋关节发育不良合并股骨上端骨折的患者采取全髋关节置换术的疗效较好。Objective To analyze the efficacy and long- term prognosis of total hip replacement in treatment of adult hip dysplasia patients with upper femur fracture. Methods A total of 61 adult hip dysplasia patients with upper femur fracture were selected and treated with total hip replacement. The efficacy was evaluated,and two lower limbs length difference,quality of life score,pain and walking conditions were observed before and after operation. Results After the treatment,the total excellent and good rate was73. 77% in 61 cases. After surgery,the Harris scores of all types of patients significantly increased（ P〈0. 05）,the ability of walking and sense of pain significantly improved（ P〈0. 05）. The length difference of the two lower limbs after operation was（ 1. 9 ± 0. 7） cm,which was significantly shorter than（ 8. 7 ± 1. 5） cm before operation（ P〈0. 05）. Patient＇ s physical function,psychological function,emotional function and social function scores after operation were significantly higher than those before operation（ P〈0. 05）. Conclusion Total hip replacement is effective in treatment of adult hip dysplasia patients with upper femur fracture.
