改良吻合血管腓骨移植治疗早期股骨头坏死

摘　　要：目的 探讨改良吻合血管腓骨移植治疗早期股骨头缺血性坏死的临床疗效。方法 随机抽取2012年5月-2015年5月我院住院治疗的25例早期股骨头坏死患者临床资料,采取改良吻合血管腓骨移植手术,记录手术时间、术中出血量及切口长度等手术指标,采用Harris评分工具评价髋关节功能恢复效果,随访并发症发生情况。结果 25例手术患者,平均游离腓骨时间（16.12±2.34）min,平均单髋手术时间（84.26±4.79）min,平均手术出血量（132.56±10.25）ml,平均（髋部）切口长度（7.44±1.09）cm,平均住院时间（8.10±0.38）d。术前Harris评分为（66.42±4.71）分,术后（87.36±5.24）分,术后评分显著高于术前（P〈0.05）。结论 改良吻合血管腓骨移植是治疗早期股骨头坏死的有效方法。Objective To evaluate the clinical results of modified free vascularized ifbular grafting （FVFG） for the treatment of osteonecrosis of the femoral head （ONFH） at early stage.Methods The clinical data of 25 cases patients with early femoral head necrosis were collected from May 2012 to May 2015 in our hospital. The operation parameters including operation time, blood loss and incision length were recorded. Harris scoring tools were used to evaluate the recovery of hip function and folow-up complications. Results In 25 patients, the mean free fibula time was （16.12± 2.34） min. The average operation time was （84.26 ± 4.79） min, the mean operative blood loss （132.56 ± 10.25） ml, the mean hip incision length （7.44 ± 1.09） cm, the average length of stay （8.10 ± 0.38） d. Preoperative Harris score （66.42 ± 4.71）, postoperative （87.36 ± 5.24） points, postoperative score was signiifcantly higher than preoperative （P〈0.05）.Conclusion Modiifed vascular ifbular graft is an effective method for the treatment of early a vascular necrosis of the femoral head.
