PFNA与PF-LCP治疗股骨粗隆间骨折临床应用及疗效分析

摘　　要：目的：对比治疗股骨粗隆间骨折中股骨近端螺旋刀片抗旋髓内钉（PFNA）固定与股骨近端解剖型锁定板（PF-LCP）固定术后的临床应用及疗效。方法选取2011年2月～2013年7月本院收治的确诊股骨粗隆间骨折患者53例，其中包括建筑从业人员、交通事故、老年人，对比观察患者临床疗效。结果临床研究结果表明抗旋髓内钉组各项指标平均手术所需时间、平均手术出血量、平均恢复所需时间及手术切口长度均优于解剖型锁定板组；根据髋关节Harris功能评分标准抗旋髓内钉组均高于解剖型锁定板组，且抗旋髓内钉组的下肢深静脉血栓（3.8%）、髋关节不适（0.0%）、髋内翻（0.0%）、应激性溃疡（3.8%）等并发症的发生率，低于解剖型锁定板组下肢深静脉血栓（3.7%）、髋关节不适（3.7%）、髋内翻（7.4%）、应激性溃疡（3.7%）等并发症的发生率，以上差异均有统计学意义（P＜0.05）。结论临床应用PFNA治疗股骨粗隆间骨折患者，具备手术切口较小、手术时间与术后恢复短、出血量小等特点，能够很好的适应各种创伤性股骨粗隆间骨折。Objective To study the comparison of clinical application and curative effect of PFNA and PF-LCP in the treatment of intertrochanteric fracture.Methods 53 patients with intertrochanteric fracture were selected from February 2011 to July 2013 in our hospital, they were construction professionals, traffc accident, the elderly, the clinical curative effect was compared.Results Clinical research results showed that average time needed for surgery, surgical blood loss, average length of time needed for recovery and surgical incision of the resistance to rotation intramedullary nailing group were superior to dissect type locking plate group. According to harris hip function score, the resistance to rotation intramedullary nailing group was higher than dissect type locking plate group, and complication rates of rotary intramedullary nailing group, deep vein thrombosis of lower limbs（3.8%）, hip discomfort （0.0%）, hip varus （0.0%） and stress ulcer （3.8%） were lower than that dissect type locking plate group, deep vein thrombosis of lower limbs （3.7%）, hip discomfort （3.7%）, hip varus （7.4%） and stress ulcer （3.7%）, the difference were statistically signiifcant （P〈0.05）. ConclusionThe clinical application of PFNA treatment of intertrochanteric fracture patients has characteristics of small incision, short operation time and postoperative recovery and the bleeding little, it could well adapt to all kinds of traumatic femoral intertrochanteric fractures.
