多维与单边外固定支架治疗胫腓骨开放性骨折的临床对比研究

摘　　要：[目的]探讨多维与单边外固定支架治疗胫腓骨开放性骨折的临床疗效。[方法]回顾性分析本院2012年1月~2014年1月诊治的53例胫腓骨开放性骨折患者的临床资料,根据应用多维外固定支架和单边外固定支架治疗分为多维组和单边组,其中多维组23例,单边组30例。所有患者固定范围均不超过膝、踝关节。所有患者均获得1年以上随访。[结果]两组术后近期疗效比较,差异有统计学意义（P〈0.05）。两组患者各发生4例钉道感染,经抗生素静滴后感染控制,单边组发生1例畸形愈合及2例骨不连,多维组发生2例踝关节活动受限。[结论]多维与单边外固定支架均是治疗胫腓骨开放性骨折的有效方法,但前者骨折愈合时间更快、后期并发症更少。[ Objective ] To investigate the clinical effect of muhi - dimensional and unilateral external fixators in the treat- ment of open tibiofibular fractures. [ Method] A retrospective analysis was performed for 53 patients with open tibiofibular frac- tures who were treated in our hospital from January 2012 to January 2014. According to the application of multi - dimensional or unilateral external fixators, the patients were divided into multi - dimensional group with 23 patients and unilateral group with 30 patients. The range of fixation did not exceed the knee and ankle for all the patients. All the patients were followed up for more than one year. [ Result] There were significant differences in the short - term outcome between two groups. Four patients in each group had pin tract infection, which was controlled by intravenous dripping of antibiotics. In the unilateral group, 1 patient expe- rienced malunion and 2 patients experienced bone nonunion, while in the multi - dimensional group, 2 patients experienced limi- tation of ankle joint motion. [ Conclusion] Multi - dimensional and unilateral external fixators are effective methods for the treat- ment of open tibiofibular fractures, but multi - dimensional external fixator has a shorter healing time of fracture and fewer late complications.
