摘  要:目的探讨急性呼吸窘迫综合征患者动脉血红细胞内外血气变化和酸碱失衡情况。方法对30例急性呼吸窘迫综合征患者和30例健康志愿者的动脉血红细胞内外的血气参数及酸碱失衡类型进行分析。结果急性呼吸窘迫综合征患者主要以酸中毒为主,包括呼吸性酸中毒及代谢性酸中毒。与正常对照组比较,病例组红细胞内酸中毒较红细胞外酸中毒变化明显,病例组的ΔPH显著低于正常对照组(P<0.05)。结论急性呼吸窘迫综合征患者主要为酸中毒,血浆PH(PeH)在7.15~7.40之间波动,红细胞内PH(PiH)变化较红细胞外显著。Objective To explore the changes of intra and extra erythrocytic blood gases and acid-base disturbance in patients with acute respiratory distress syndrome. Methods The changes of blood gases of intra and extra erythrocytic blood gases and the types of acid-base disturbance of 30 patients with acute respiratory distress syndrome and 30 healthy subjects were analyzed. Results Acute respiratory distress syndrome patients mainly had acid poisonging distress, including the respiratory acidosis and metabolic acidosis. The intra-erythrocytic PH (P^H) was more obvious than exotra-erytbrocytic PH in patients with acute respiratory distress syndrome. Compared with normal control group, the ApH of patients with acute respiratory distress syndrome was significantly lower (P 〈0.05). Conclusions Acute respiratory distress syndrome patients mainly have acidosis. The plasma PH value fluctuates between 7.15 - 7.40. The change of intra-erythrocyte PH is more significant than that ofextra-erythrocyte PH.
