摘  要:目的探讨老年甲状腺功能异常状态下糖、脂代谢特征及其与氧化应激的关系。方法收集我院2014年1月至2015年7月收治的老年甲功异常患者100例,包括甲亢、甲减各50例;同时选择健康对照组50例。均行糖耐量试验,并测定各组血脂、丙二醛(MDA)、超氧化物歧化酶(SOD)、氧化型低密度脂蛋白(OX-LDL)水平。结果甲亢组患者的MDA、SOD显著高于对照组(P<0.01),SOD/MDA、OX-LDL显著低于对照组(P<0.01)。甲减组患者的MDA、OX-LDL显著高于对照组(P<0.01),SOD显著高于对照组(P<0.05),SOD/MDA显著低于对照组(P<0.01)。甲亢组患者1h PG、2h PG显著高于对照组(P<0.01),TC、LDL-C、TC、LDL-C显著低于对照组(P<0.01)。甲减组患者TC、TG、LDL-C显著高于对照组(P<0.01),HDL-C显著低于对照组(P<0.05)。结论老年甲亢患者糖耐量降低,且脂代谢水平亢进;老年甲亢患者氧化应激的发生可能与糖、脂代谢紊乱有关。老年甲减患者糖耐量无异常,但脂代谢水平降低;老年甲减患者氧化应激的发生可能与脂代谢紊乱有关。Objective To investigate the glycolipid metabolism and its relationship with oxidative stress in elderly patients with thyroid dysfunction condition. Methods A total of 100 cases of elderly patients with abnormal thyroid function admitted to our hospital from January 2014 to July 2015 were selected, including 50 cases of hyperthyroidism and 50 cases of hypothyroidism; 50 cases of healthy person were selected as the control group. All cases received tolerance tests, and the levels of blood lipids, malondialdehyde (MDA), superoxide dismutase (SOD), oxidized low-density lipoprotein (OX-LDL) were detected and compared. Results The levels of MDA and SOD of patients with hyperthyroidism were significantly higher than those of control group (P 〈0.01), and the levels of SOD/MDA and OX-LDL were significantly lower than those of control group (P 〈0.01). The levels of MDA and OX-LDL of patients with hypothyroidism were significantly higher those of control group (P〈0.01), the level of SOD was significantly higher than that of control group (P〈0.05), and the level of SOD/MDA was significantly lower than that of control group (P 〈0.01). The levels of lhPG and 2hPG of patients with hyperthyroidism were significantly higher than those of control group (P 〈0.01), the levels of TC, TG, LDL-C and HDL-C were significantly lower than those of control group (P 〈0.01). The levels of TC, TG and LDL-C of patients with hypothyroidism were significantly higher those of control group (P〈0.01), and the level of HDL-C was significantly lower than that of control group (P〈0.05). Conclusions Elderly patients with hyperthyroidism have lipid metabolism hyperthyroidism and reduced glucose tolerance; Oxidative stress in elderly patients with hyperthyroidism may occur with glucose and lipid metabolism disorders.
