塞来昔布联合正清风痛宁片对类风湿关节炎的疗效分析

摘　　要：目的:探讨塞来昔布联合正清风痛宁片对类风湿关节炎患者外周淋巴细胞的影响及疗效。方法:类风湿关节炎患者122例,随机分为两组:对照组57例,单用塞来昔布治疗;观察组65例,塞来昔布联合正清风痛宁片治疗,疗程4周。比较两组患者治疗前后IL-1、IL-6、TNF-α水平及类风湿因子(RF);肿胀疼痛关节数、VAS疼痛评分及疗效。结果:(1)总有效率观察组高于对照组(66.15%vs.52.63%,P<0.05);(2)治疗后观察组RF、IL-6、TNF-α水平均低于对照组[(19.27±3.81)vs.(36.64±4.11)]U/m L、[(10.34±1.64)vs.(14.37±3.21)]ng/L、[(8.54±2.47)vs.(12.58±1.69)]μg/L,均P<0.05;(3)治疗后观察组与对照组比较,肿胀疼痛关节数差异有统计学意义(P<0.05),但VAS疼痛评分比较差异无统计学意义(P>0.05)。结论:塞来昔布联合正清风痛宁片治疗类风湿关节炎,能够显著改善患者症状,缓解疼痛,减轻炎症反应,其疗效优于单用塞来昔布治疗。Objective： To investigate the influence of celecoxib combined with Zhengqingfengtongning tablets on inflammatory cytokines and assess the clinical efficacy in patients with rheumatoid arthritis. Methods： 122 cases of patients with rheumatoid arthritis were randomly divided into two groups,65 patients in the treatment group were treated with celecoxib plus Zhengqingfengtongning,while57 patients in the control group were only administered with celecoxib orally. After 4 weeks of treatment,the levels of IL-1,IL-6,TNF-αand rheumatoid factor,the number of joint swelling and pain,VAS pain scores and the overall treatment effective rate were compared between the two groups before and after treatment. Results： RF,IL-6,TNF-α in the treatment group were significantly lower than those in the control group after treatment（ 19. 27 ± 3. 81 vs. 36. 64 ± 4. 11 U / m L,10. 34 ± 1. 64 vs. 14. 37 ± 3. 2 ng / L,8. 54 ± 2. 47 vs. 12. 58 ±1. 69 μg / L,all P〈0. 05）. After treatment,the number of joint swelling and pain of the treatment group was significantly lower than that of the control group（ 4. 24 ± 3. 51 vs. 15. 63 ± 3. 43,P〈0. 05）; the VAS pain scores in treatment group was significantly lower than that in the control group（ 1. 23 ± 3. 24 vs. 8. 19 ± 2. 26,P〈0. 05）. The total effective rate of the treatment group was higher than that of the control group（ 66. 15% vs. 52. 63%,P〈0. 05）. Conclusion： Celecoxib combined with Zhengqingfengtongning tablets in the treatment of RA patients can significantly improve symptoms,relieve pain,reduce inflammation,which is more effective than a single treatment with celecoxib.
