摘  要:目的:利用能谱CT基物质成像技术测量活体甲状腺碘含量的正常值,并对其准确性进行研究。方法:对76例疑颈部或甲状腺疾病患者进行能谱CT扫描。利用GSIViewer浏览器处理,取得甲状腺组织的最佳对比噪声比对应的单能量值,测量甲状腺的碘密度值、水密度值。另外收集35例结节性甲状腺肿手术离体标本,选取结节旁正常甲状腺组织,利用砷铈催化分光光度法测量标本碘含量,并分别研究其与碘密度值、水密度值的相关性。结果:男性与女性的碘密度值分别为(1.57±0.58)mg/m L、(1.39±0.64)mg/m L,差异有统计学意义(P〈0.05)。除40-60岁组与〉60岁组比较碘密度值差异无统计学意义外(P〉0.05),其余各组比较均有统计学意义(P〈0.05和P〈0.01)。结节性甲状腺肿组的离体标本最佳Kev碘密度值(0.28-2.31)mg/m L,平均(1.38±0.56)mg/m L,与离体标本碘含量[(0.173-0.64)mg/g,平均(0.357±0.064)mg/g]有相关性(r=0.820,P〈0.01);CT值为58.12-129.78 HU,平均(94.02±20.37)HU,与离体标本碘含量有相关性(r=0.754,P〈0.01);水密度值为(1 040.73-1 067.17)mg/m L,平均(1 055.93±11.16)mg/m L,与离体标本碘含量未见相关(r=0.064,P〉0.05)。结论:宝石能谱CT成像可更直接、更准确地测量活体甲状腺碘浓度,从而为诊断甲状腺病变提供参考。Objective: To measure the iodine concentration of the living the normal value base on material imaging measurement,and to study its accuracy. Methods: Total of 76 cases of suspected cervical or thyroid disease underwent gemstone spectral imaging. Then the imagine data were transmitted to AW 4. 4 station to obtain monochromatic images of optimal contrast to-noise ratio( CNR) and iodion-based images with GSI Viewer. Iodine,water concentration of ROl in thyroid tissue was measured. In addition,35 cases of surgical specimens of nodular goiter were collected,and select normal thyroid tissue of adjacent nodules. The iodine content of samples was measured by As-Ce-Catalytic spectrophotometry. Relationship between iodine and water concentration with it were analyzed. Results: The difference of iodine concentration between male and female patients was statistically significant( P〈0. 05). There was no statistical difference between 40- 60 group and 60 group( P〈0. 05),while differences between other groups were statistically significant( P〈0. 05 and P〈0. 01). The comparison among groups of water concentration were not statistically significant( P〈0. 05).In nodular goiter group,the iodine concentration of isolated specimen is 0. 173-0. 64 mg / g,mean( 0. 357 ± 0. 064) mg / g. Statistical analysis showed correlation( r = 0. 820,P〈0. 01) between iodine concentration( 0. 28- 2. 31 mg / m L,mean( 1. 38 ± 0. 56) mg / m L)and the iodine concentration of isolated specimen. There was no correlation( r = 0. 064,P〈0. 05) between water concentration( 1 040. 73- 1 067. 17 mg/m L,mean( 1 055. 93 ± 11. 16) mg/m L) and the iodine concentration of isolated specimen. Conclusion:Gemstone spectral imaging can be more direct and more accurate quantifying of iodine concentration of normal thyroid. This can provide reference for the diagnosis of thyroid diseases.
