幽门螺杆菌对2型糖尿病患者糖脂代谢的影响

摘　　要：目的通过分析2型糖尿病(T2DM)患者幽门螺杆菌(Hp)的感染情况,探讨Hp和T2DM患者脂代谢的关系。方法 195例T2DM患者,通过胃黏膜活检或13 C尿素呼气试验确定Hp感染率,并检测常见临床指标糖化血红蛋白(HbA1c)、空腹血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、尿微量白蛋白(ALB)、血清C肽、胰岛素、24小时尿蛋白,测量入院血压,并计算体质量指数(BMI)及胰岛素抵抗指数(HOMR-IR)。结果 T2DM患者Hp感染率为48.2%(94/195),对照组Hp感染率37.6%(88/234)(P<0.05)。糖尿病患者Hp阳性组中FBG、TG、ALB、24小时尿蛋白、血清胰岛素、HOMR-IR均高于Hp阴性组,血清C肽低于Hp阴性组(P均<0.05)。结论糖尿病患者Hp感染率高,Hp感染加重糖尿病患者糖脂代谢紊乱,加重胰岛素抵抗。根治Hp可改善糖尿病患者糖脂代谢,降低尿蛋白,在一定程度上可预防和延缓糖尿病并发症的发生及发展。Objective Through the analysis of type 2diabetes mellitus（T2DM）patients with helicobacter pylori（Hp）infection status,to investigate the correlation between Hp and glucolipid metabolism in T2 DM.Methods The study enrolled 195 cases of patients with T2 DM who enderwent by gastroscope gastric mucosa biopsy or 13 C urea breath test to determine Hp infection rate,and detections were performed in the common clinical indicators such as HbA1 c,FBG,TG,TC,HDL-C,LDL-C,ALB,C peptide,insulin,24-hour urine protein,admission blood pressure,BMI and HOMR-IR.Results Hp infection rate in diabetes group was 48.2%（94/195）,but that in control group37.6%（88/234）（P〈0.05）.In diabetic patients with Hp positive group,FBG,TG,ALB,24 hours urinary protein,serum insulin,HOMR-IR were higher than in Hp negative group,serum c-peptide was lower than in Hp negative group（all P〈0.05）.Conclusion Hp infection rate in diabetic patients is higher,Hp infection may increase diabetes glucolipid metabolic disorders and insulin resistance.Eradicating Hp can improve the glucolipid metabolism in diabetic patients,reduce urinary protein,to a certain extent,can prevent and delay the occurrence and development of diabetes complications.
