摘  要:目的 为了提高对儿童髓母细胞瘤影像诊断的准确率,主要探讨儿童髓母细胞瘤的DWI表现。方法 回顾性分析了72例儿童髓母细胞瘤的MRI影像资料。其中男44例,女28例,发病年龄1~18岁。72例均进行了DWI扫描,且全部经手术、病理证实。结果 72例髓母细胞瘤中61例位于小脑蚓部,11例位于小脑半球;MR扫描呈等或稍长T1稍长T2信号,不同程度的不均匀强化。DWI(b=1 000)扫描65例肿瘤实性部分呈高信号,7例肿瘤实性部分呈等信号。影像报告诊断正确率为74%。结论 儿童髓母细胞瘤有其典型的好发部位及特征性影像表现,掌握其影像学特点及鉴别诊断要点,完善检查项目,特别是加做磁共振弥散加权成像扫描,根据肿瘤DWI的表现,可以提高影像诊断的准确率,降低误诊率。Objective To study diffusion weighted imaging features of medulloblastoma in children. Methods Imaging findings of 72 children with medulloblastoma were retrospectively reviewed. The patients included 44 boys and 28 girls, ranging in age of onset from 1 to 18 years. All the 72 patients were examined with pre-contrast and post-contrast T1 weighted MR imaging, non-contrast T2 weighted MR imaging, and diffusion weighted imaging. Medulloblastoma was confimed by pathologic examinations in the 72 patients. Results Among 72 cases with melulloblastoma, 61 cases' lesions were located at the cerebellar vermis, and other 11 cases' lesions were in the cerebellar hemisphere. All cases showed iso-signal or low-signal on both T1 weighted MR images and high-signal on T2 weighted MR images. And all cases were enhanced at different extension. The precision rate of imaging diagnosis report was 74%. Conclusion Medulloblastoma in children has a typical predilection in location and characteristic imaging findings. Knowing well the imaging findings of medulloblastoma and useing diffusion weighted imaging may help us to improve imaging diagnostic accuracy.
