摘  要:目的探讨磁共振波谱成像(magnetic resonance spectroscopy,MRS)对鞍旁海绵状血管瘤和鞍旁脑膜瘤的鉴别诊断价值。方法回顾性分析经证实的6例鞍旁海绵状血管瘤和10例鞍旁脑膜瘤患者磁共振常规扫描、MRS及增强扫描影像资料进行分析。结果常规和增强扫描在二者鉴别上困难;鞍旁海绵状血管瘤波谱特点为无N-乙酰天门冬氨酸(N-acetyl aspartate,NAA)、胆碱(Choline,Cho)和肌酸(Creatine,Cr)等峰,只见明显脂质(Llipid,Lip)峰;鞍旁脑膜瘤无NAA、Cr等峰,均见明显Cho峰,3例可见丙氨酸峰(Alanine,Ala),两者MRS表现具有明显差异性。结论 MRS在鞍旁海绵状血管瘤与鞍旁脑膜瘤的鉴别诊断中具有较高的价值。Objective To explore the value of WRS in the differential diagnosis of saddle cavernous hemangioma and meningioma. Methods The imaging data of 10 patients with saddle cavernous hemangioma and 6 patients with meningioma, who underwent the convertional MRI scanning, MRS, and enhanced scanning, were retrospectively analyzed in this work. Results It was difficult to identify the both tumors with conventional MRI scanning and enhanced scanning; The spectral characteristics of the cavernous angioma beside sella tursica were the peaks without the N-acetyl aspartate, Choline, Creatine peaks, and so on, but, with L-lipid peak clearly. The spectral characteristics of the meningioma beside sella tursica were peaks without N-acetyl aspartate and Creatine peak, but, with Choline peak clearly. Alanine peak can be seen in 3 cases and there were marked differences of MRS in both tumors. Conclusion MRS has high value in the differential diagnosis of saddle cavernous hemangioma and meningioma
