摘 要:目的探讨低剂量自适应迭代重建技术(AIDR3D)在头颈部CTA扫描中对图像质量及辐射剂量的影响。方法行头颈部CTA检查的患者80例,随机数字表法分为A、B两组,每组40例。A组的扫描参数为120k V、300m A,经滤波反投影法(FBP)重建。B组为100k V、自动毫安,经AIDR3D迭代重建。对两组图像的噪声、信噪比(SNR)、对比噪声比(CNR)、图像质量主观评分及辐射剂量进行评价。结果 1)客观评价:颈总动脉血管噪声值B组低于A组,差异有统计学意义(P<0.05)。颈动脉窦及颈内动脉血管噪声值B组高于A组,差异有统计学意义(P<0.05)。颈部SNR及CNR值B组高于A组,差异均有统计学意义(P<0.05)。颅内段血管(MCA1)噪声值A组低于B组,差异有统计学意义(P<0.05)。两组间颅内血管SNR及CNR差异无统计学意义(P>0.05);2)主观评价:颈总动脉评分B组高于A组,差异有统计学意义(P<0.05)。两组间颈动脉窦及颈内动脉评分差异均无统计学意义(P>0.05)。颅内动脉评分B组低于A组,但B组图像质量均能满足临床诊断要求;3)两组有效辐射剂量B组明显低于A组,差异有统计学意义(P<0.05),B组较A组辐射剂量降低约71%。结论在头颈CTA联合扫描中,使用100k V和自动毫安调节,并运用AIDR3D迭代重建技术,在获得优质血管图像的同时可明显降低辐射剂量。Objective To assess the:effect of adaptive iterative dose reduction 3D (AIDR3D) on image quality and radiation dosage in CT angiography of craniocervical arteries. Methods 80 patientswho underwent the head and neck CTA examination were divided into Group A and Group B at random. Each group included40 patients. The patients in group A were scanned using 120 kV, 300 mA, and images were' reconstructed using conventional FBP. The other group of patients were scanned using 100 kV, with Auto-mA technology and reconstructed using A1DR3D. All of the images were evaluated on image quality and radiation dose. Results 1 ) Objective evaluation : The noise level of group B was significant lower than group A in the common carotid artery ( P 〈 0.05 ), but higher in carotid sinus and internal carotid artery ( P 〈 0.05 ). Neck artery SNR and CNR of group B were significant higher than group A ( P 〈 0.05 ). In the evaluation of intracranial arteries( MCA1 ), the noise level of group A was lower than group B ( P 〈 0. 05 ), but there was no statistical difference in SNR and CNR between the two groups ( P 〉 0. 05 ) ; 2) Subjective evaluation: the image quality score of common carotid artery in group B was higher than group A ( P 〈 0. 05 ) , but there was no difference in image quality scoring of carotid sinus and internal carotid artery ( P 〉0. 05 ). The score of group B was lower than group A in image quality of intracranial artery, but the quality of all the images could meet the requirements of clinical diagnosis; 3) Compared with group A, radiation dose in group B was reduced by 71%. There was significant difference ( P 〈 0.05 ). Conclusion For CT angiography of craniocervical arteries, AIRD3D reconstruction along with 100 kV and auto-mA modulation produces diagnostic quality images while substantially reducing radiation dose.
