摘  要:目的探讨在微波消融治疗肝癌中,采用低剂量CT进行引导的可行性。方法 100例次经肝动脉介入栓塞治疗后的患者,按照引导CT剂量分为常规组(管电流100m A)50例和低剂量组(管电流50m A)50例,进行微波消融治疗。分别记录和比较容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)和有效剂量(ED)。结果两组患者的CT图像均能满足微波消融治疗要求,均顺利完成治疗。低剂量组患者总辐射剂量(10477±4120)m As vs(5459±933)m As,P<0.01、CTDIvol(229±98)m Gy vs(78±14)m Gy,P<0.01、DLP(2094±891)m Gycm vs(78±14)m Gycm,P<0.01、ED(31.4±13.4)m Gy vs(16.4±2.8)m Gy,P<0.01,均较常规组明显降低。结论对于经肝动脉介入治疗后的肝癌患者进行微波消融治疗,可以采用50m A管电流低剂量CT引导治疗。Objective To explore of feasibility of low-dose CT scan in microwave ablation in patients with hepatocellular carcinoma. Methods According to CT-guided dose, 100 case-time patients after transcatheter arterial chemoembolization were randomly divided into convention dose group (tube current 100 mA) and lowe-dose group (tube current 50 mA). The patients received microwave ablation treatment. The total radiation dosage, CTDIvol, DLP and ED were recorded and compared in two groups. Results The CT images can meet the demand. The CT-guided microwave ablation was completed. The total radiation dosage (10477±4120) mAs vs (5459±933) mAs, P 〈0.01, CTDIvol (229±98) mGy vs (78 ±14) mGy, P 〈0.01, DLP (2094 ±891 ) mGycm vs (78 ±14) mGycm, P 〈0.01 and ED(31.4 ±13.4) mGy vs ( 16.4 ±2.8) mGy, P 〈 0.01 in lowedose group were reduced obviously than that in convention dose group. Conclusion To the microwave ablation treatment of the patients after transeatheter arterial chemoembolization, the low-dose CT-guided treatment can be used.
