摘  要:目的探讨经会阴实时三维超声在女性压力性尿失禁(SUI)患者中的应用价值。方法应用经会阴实时三维超声技术,观察36例女性SUI患者在静息和Valsalva动作(屏气并向下用力至最大腹压)两种状态下超声图像变化情况,并分析SUI患者平静呼吸状态下的尿道长度、膀胱逼尿肌厚度(DWT);最大Valsalva动作后的膀胱颈下降值(BND)、膀胱尿道后角、尿道倾斜角及有无尿道内口漏斗形成等指标,以42例正常女性作为对照。结果静息状态下,SUI组与对照组尿道长度及DWT比较,差异无统计学意义(P>0.05);最大Valsalva动作后,SUI组的膀胱颈下降值明显大于对照组,尿道内口漏斗形成率也明显高于对照组(P<0.05),膀胱尿道后角及尿道倾斜角增大概率也高于对照组(P<0.05)。结论经会阴实时三维超声能够对压力性尿失禁患者做出典型准确的超声图像特征评价,准确的评估盆底解剖及功能,对临床推广应用价值很大。Objective To explore application value of real-time three-dimensional ultrasound via perineum in the evaluation of female patients with stress urinary incontinence (SUI). Methods By means of real-time three-dimensional ultrasound via perineum, we observed the ultrasound image changes of 36 female patients with SUI both at rest and Valsalva maneuver ( holding the breath to reach the maximum abdominal pressure) and analyzed the urethra length and detrusor thickness (DWT) at quiet breathing state as well the bladder neck descent value, posterior urethra - vesical angle, angle of inclination of urethra, whether or not the funnel formation in internal urethral orifice at Valsalva maneuver state, with 42 normal women as controls. Results At the resting state, the urethral length and DWT of patients with SUI were not statistically significant different compared with those of controls ( P 〉 0. 05 ). At the maximum Valsalva maneuver, bladder neck drop value of patients with SUI was significantly greater than that of controls and, also, the rate of funnel formation in internal urethral orifice was significantly higher ( P 〈 0.05 ). The probability of the increase in posterior urethra-vesieal angle and angle of inclination of urethra was markedly higher ( P 〈 0. 05 ). Conclusion The real-time three-dimensional ultrasound via perineum is able to make an accurate assessment of ultrasound image traits of patients with SUI as well as an accurate assessment of pelvic anatomy and function, which possesses great value in clinical application.
