非瓣膜性房颤患者经皮左心耳封堵术的围手术期护理

摘　　要：目的:总结经皮左心耳封堵术预防非瓣膜性房颤并发脑卒中的围手术期护理经验。方法:对本院2015年3月至2016年3月8例经皮左心耳封堵术治疗非瓣膜性房颤合并脑卒中患者的研究资料进行回顾性研究,所有患者术前、术中生命体征及术后的护理,并在术后给予抗凝药物等,以及对可能出现并发症的护理,总结围手术期护理经验。结果:8例患者均顺利完成经皮左心耳封堵术,术后24h行经超声心动图检查,8例患者均未出现心包积液;术后随访3个月后,8例患者均未出现死亡等其他相关事件的不良心脏事件。结论:在经皮左心耳封堵术围手术期,有效护理有助于心房颤动合并脑卒中患者术后的恢复,对降低术后并发症和提高手术成功率有着极其重要的意义。[Objective]To summarize the percutaneous left atrial appendage occlusion to prevent perioperative nursing experience of non valvular atrial fibrillation stroke,in order to improve the quality of life of patients provides the basis.[Method]The nursing of 8 cases of our hospital from March 2015 to 2016 March percutaneous left atrial appendage occlusion for treatment of non valvular atrial fibrillation patients with stroke were retrospectively studied,before surgery,intraoperative vital signs and postoperative nursing,close observation of the patient vital signs and postoperative give anticoagulant drugs and drug administration,and complications,summarize the percutaneous left atrial appendage occlusion to prevent perioperative nursing experience of non valvular atrial fibrillation stroke. [Result]Eight cases of non valvular atrial fibrillation complicating stroke patients were successfully completed by percutaneous left atrial appendage occlusion and recovered well after operation,postoperative 24 h by ultrasound heartbeat graph examination,no pericardial effusion was observed in 8 patients; followed up for 3 months after operation,8 patients did not appear death,malignant arrhythmia and acute cardiac tamponade,cerebral vascular accident and other adverse cardiac events. [Conclusion]In percutaneous left atrial appendage occlusion during perioperative period,effective nursing could help patients with atrial fibrillation after stroke recovery,and it was very important to reduce postoperative complications and improve the success rate of operation.
