阻塞性睡眠呼吸暂停综合征患者左心室质量指数的相关因素分析

摘　　要：目的探讨阻塞性睡眠呼吸暂停综合征(OSAS)患者血清基质金属蛋白酶9(MMP-9)及金属蛋白酶组织抑制因子1(TIMP-1)水平与左心室质量指数(LVMI)的相关性。方法 OSAS患者60例,根据呼吸暂停低通气指数(AHI)分为轻中度OSAS组(5≤AHI≤30h-1)及重度OSAS组(AHI>30h-1),另外收集门诊健康体检者20例作为正常对照组。分别测定3组的血清MMP-9及TIMP-1浓度,行心脏彩超检查计算LVMI。结果重度OSAS组的LVMI及血清MMP-9、TIMP-1浓度均高于正常对照组及轻中度OSAS组,差别均有统计学意义(P<0.05)。多元线性回归分析显示,高血压病史、体质量指数(BMI)、AHI及血清MMP-9、TIMP-1水平均是OSAS患者LVMI的影响因素(均为P<0.05)。结论OSAS可能是左心室肥厚的独立危险因素,而血清的MMP-9及TIMP-1浓度对评估OSAS患者早期的心室重塑可能具有一定参考价值。Objective To investigate plasma matrix metalloproteinase-9 （MMP-9） and tissue inhibitor of metalloproteinase-1（TIMP-1）in relationship with left ventricular mass index（LVMI） in patients with obstructive sleep apnea syndrome（OSAS） . Methods Sixty OSAS patients who met thediagnosis criteria were divided into 2 groups according to apnea hypopnea index （AHI）：mild to moderate group （5≤AHI≤30 times/h） and severe group（AHI〉15 times/h） . Twenty healthy individuals were included as contro1 cases . Left ventricular mass index（LVMI）were examined ,and plasma MMP-9 and TIMP-1 were detected . Results The LVMI and the levels of plasma MMP-9 and TIMP-1 were much higher in severe group than those in the mild to moderate group and the control group（P〈0 .05） . Multi-linear regression analysis indicated that AHI ,plasma MMP-9 and TIMP-1 had influence on LVMI in OSAS patients . Conclusion OSAS may be an independent risk factor for the left ventricular hypertrophy ,and the levels of plasma MMP-9 and TIMP-1 can be usedas a reference for assessment of early ventricular remode-ling in OSAS patients .
