摘　　要：文章就腰椎间盘突出症(LIDP)的主要治疗性手法与慎用手法进行了探讨。认为:(1)肌肉痉挛、神经根及其周围软组织炎症、腰椎关节僵直和神经根受到周围组织的挤压与碰撞以及可能合并存在的腰椎小关节紊乱等3个方面是引起LIDP临床症状的主要病理环节,针对上述病理环节,与之相应地,借助油性介质下直接在腰背部皮肤上的掌推与双手拇指平推法、拳扌衮法,以及腰椎棘突上的掌根弹压法、拇指斜推棘突法、直腿抬高牵伸扳腿法、以牵伸为主的单腿后伸扳法等手法,则是LIDP的主要治疗性手法。(2)被动直腿扳腿法,腰椎旋转扳法与斜扳法及踩跷法,以及用力方向不当的肘压法等手法,有可能促使病情加重或出现意外不良事件,应视为LIDP的主要慎用手法。In this article,the main therapeutic methods and careful manipulation on lumbar intervertebral disc protrusion（ LIDP） were discussed. The author believes：（ 1） The main pathological causes of LIDP are as the following 3 aspects： muscle cramps,nerve root and its surrounding soft tissue inflammation; lumbar stiffness,and the nerve root is subjected to the extrusion and collision of the surrounding tissue; the possibility of the existence of lumbar facet joint disorder. In view of the above 3 main pathological causes,the main therapeutic methods should be with the aid of oil medium pushing the back skin directly by palms,horizontal pushing by thumbs and fist rolling; palm root elastic pressing method on lumbar spine,inclined pushing spines process by thumbs,straight leg-raising and pulling method,single leg back stretching method. These are the main manipulations for LIDP.（ 2） Passive straight leg pulling method,lumbar rotation pulling method,lumbar oblique pulling method,stepping lumbus and elbow pressure method with improper force direction and so on,would likely lead to the disease development or the occurrence of unexpected adverse events. These techniques should be regarded as the main careful manipulations.
