外侧入路交锁髓内钉固定配合中药治疗胫骨骨折

摘　　要：目的：观察采用胫骨结节外侧入路交锁髓内钉固定治疗胫骨骨折的临床疗效。方法：将37例胫骨骨折采用胫骨结节外侧入路实施交锁髓内钉固定。术后通过辨证论治予中药汤剂内服。记录手术时间、失血量、下地负重时间、骨折愈合时间及相关并发症进行分析,患者肢体功能参照Johner-Wruhs标准进行评估。结果：37例均获随访,时间12~48个月。手术时间：（40±20）min;失血量：（50±30）m L;下地负重时间：（7±4.8）d;骨折愈合时间：（16±5.6）周。本组病例均获得骨折愈合。并发症：远端锁钉脱落1例,近端锁钉钉尾皮下滑囊炎2例,膝前区疼痛2例。结论：胫骨结节外侧入路交锁髓内钉固定治疗胫骨骨折获得良好的临床疗效,在具有髓内钉固定诸多优点的同时显著降低膝前区疼痛的发生。Objective：To observe the clinical effect of the treatment of tibial fractures with interlocking intramedullary nail fixation for lateral approach of tibial node. Methods：37 cases of tibial fractures were treated with interlocking intramedullary nail. After surgery,through dialectical treatment,oral administration of traditional Chinese medicine was offered. The operation time,blood loss,time of loading,fracture healing time and related complications were analyzed,and the limb function of the patients was evaluated according to Johner-Wruhs standard. Results：All the 37 cases were followed up for 12 to 48 months. Operation time：（40±20）min;blood loss：（50±30）m L;under the loading time：（7±4.8）d;fracture healing time：（16±5.6）weeks. Fracture healing was achieved in all the cases. Complications：distal locking nail loss in 1 cases,proximal interlocking nail tail subcutaneous bursitis in 2 cases,2 cases of anterior knee pain. Conclusion：The treatment of tibial fractures with interlocking intramedullary nail fixation for lateral approach of tibial node has a good clinical effect,which can significantly reduce the incidence of pain in the anterior region of the knee with the advantages of intramedullary nail fixation.
